Job Summary: The Licensed Practice Nurse (LPN) Navigator utilizes a high level of knowledge of the disease process in the coordination of critical medical information for the specialty department. The Navigator works directly with physicians, associate practitioners, and nursing staff to effectively communicate medical issues. Coordinates care with referring providers inside and outside of Kaiser Permanente, including hospital systems, radiology, imaging studies, surgical procedures, and external physicians. Reviews data received prior to the visit and utilize standing orders, workflows, specialty guidelines, and sound judgment for timely scheduling of appointments that are in line with the treatment plan. Demonstrates the ability to be self-directed and act independently to manage multiple scheduling tasks at once and intervene as appropriate to resolve issues. Essential Responsibilities: Utilize direct template management in the daily coordination of member appointments. Ensures that members are scheduled with the appropriate subspecialist within a department. Serves as a resource for internal and external providers who may have STAT or urgent referrals. Asks pertinent questions to ascertain need for office visit and assist in scheduling appointments, if necessary, based on guidelines established by the department providers. Facilitate member appointments as triaged by the primary physician or associate practitioners, lab, radiology, or specialty consults for follow-up encounters. Ensure that various functions, outpatient schedules, procedural schedules, imaging are coordinated and efficient. Assist with the day-to-day management of new patient referrals from internal and external providers, as well as self-referrals. Act as a resource for the call center and advice for appropriate scheduling of members within a department. Responsible for new patient consult research of external medical records, procedures, pathology and radiology reports and lab test results. Maintains and updates medical record files, specialty charts, and completes disability forms. Responsible for communicating with external referring providers, patients, and family members to obtain necessary medical records, to include imaging films and reports, CT Scans, pathology reports, operative notes, office visit notes, lab data, and patient consult notes, and making them available for the provider interview during a new consult visit. Responsible for maintaining accurate record keeping, referral database and generating reports on referral activity. Consults with Practitioners, RNs, and Nursing Supervisor to provide information relating to the department. May arrange hospital admissions under the supervision of a RN. Answers inquiries from external/internal providers, HCT members, patients and family members by telephone or in person relating to the information needed in the referral process. Participates in departmental QA activities; assists supervisor in compliance with regulatory agencies, development and revision of general procedures and standards. Maintains statistical data as requested by provider or supervisor. Responsible for receipt of results and reviewed results with the timing of the consult visit. Carry out various direct patient care activities within scope of practice and accurately documents patient care in the medical record according to KP and department policy. Assist with the maintenance and revision of clinic templates. Assists in orientation of staff as needed. May perform other duties as assigned.

Participates in departmental orientation, training, and competency validation of new staff (after completion of validator training). Leads departmental or regional performance improvement initiatives (access, quality, service, people). Autonomously perform procedures under provider supervision and evaluate patients response during and after the procedure. Serves as a preceptor, resource, and mentor for new members of the healthcare team and nursing students. Provide at least one (1) training in-service each year. Job Description Assesses, determines, and coordinates placement of all admissions (elective and emergent) and transfers in accordance with clinical standards and guidelines. Ensures patients are placed in clinically appropriate level of care via established algorithms and protocols. Upholds standards of systemwide customer service program. This role is PRN with a minimum of 4 shifts per six weeks. This will be variable shifts (7AM-7PM, 10AM-10PM, 11AM-11PM). Some weekends required. Experience 5 years of experience in nursing 2 years of experience in critical care with advanced clinical assessment skills Preferred Qualifications Bachelor of Science in Nursing Experience in progressive leadership and training as house supervisor Previous PICU, ED preferred Epic Microsoft Teams, Cisco and RedCap expertise Education Graduation from an accredited school of nursing; Bachelor of Science in nursing preferred Certification Summary Licensure as a Registered Nurse in the single State of Georgia or Multi-State through the Enhanced Nurse Licensure Compact Basic Life Support (BLS) within 30 days of employment Pediatric Advanced Life Support (PALS) within 90 days of employment Knowledge, Skills, and Abilities Ability to manage multiple and continually changing priorities and make rapid decisions Demonstrated ability in integrating pathophysiology, psychology, and sociology into the pediatric clinical setting Effective verbal and written communication skill and ability to handle conflict and reach consensus and collaboration Must have computer experience and proficiency in a wide variety of software, e.g.., Microsoft Office applications and database management Must be able to successfully pass the Basic Windows Skill Assessment at 80% or higher rating within 30 days of employment Job Responsibilities Coordinates patient placement activities via Transfer Center to optimize patient flow, patient outcomes, admission appropriateness, and cost efficiencies in support of organizational strategic initiatives while working with campus house supervisors, physicians, and nursing staff. Maintains assigned databases. Collaborates in collection of data for identification and resolution of flow issues. Collects, analyzes, and interprets outcome data systematically to identify problem areas and assess need for improvement and/or support changes in systemwide processes in relations with Transfer Center. Prepares periodic reports as directed. Participates actively and contributes to organizational initiatives that impact patient flow. Supports continuous performance and quality improvement. Communicates openly with appropriate leadership team to ensure knowledge of key decision factors. Serves as expert clinical resource for patient placement by ensuring smooth functioning of Transfer Center process to reduce wait times and avoid unnecessary patient movements. Maintains positive working relationship and demonstrates excellent customer services skills in all interactions between Transfer Center and physicians, internal customers, and external customers. Supervises Bed Placement Coordinator. Empowers, promotes, and develops team members by fostering an environment which promotes professional development and problem-solving, maintaining and enhancing knowledge of current issues and integrating ideas into practice. Assists with Transfer Center hiring process. Assists with orientation and continuing education of peers.
