Licensed Practical Nurse (LPN)

Overview The Department of Internal Medicine at Macon & Joan Brock Virginia Health Sciences at Old Dominion University is seeking a Licensed Practical Nurse (LPN) to join the team. Provides nursing care for patients and assisting physicians. Clinical and administrative duties include: Triage patients [vital signs, height/weight, medical/social history, medication reconciliation] Stock/prepare exam rooms Retrieve imaging and laboratory results Process work task/message ques in the electronic health records [EPIC], and the patient portal [myChart] Ability to work in our clinical and administrative areas with the Department of Medicine. Responsibilities Perform patient intake including obtaining vital signs, reconciling medications and allergies, documenting chief complaints, administering medications and immunizations according to approved protocols, observing for adverse reactions, assisting providers with examinations, minor procedures, diagnostic testing, and implementing standing orders. Provide basic patient care and document all nursing interventions in the electronic medical record. Manage patient-related tasks and follow-up activities including responding to patient messages and telephone calls, communicating test and laboratory results, answering patient questions, coordinating referrals and specialist appointments, interacting with family members and healthcare providers, documenting all patient communications, notifying providers of critical results, and ensuring same-day response to patient inquiries when appropriate. Review charts for completeness, monitor test results and task completion, reconcile discharge medications, manage prescription refills, obtain medication prior authorizations, administer PPD testing as needed, and ensure accurate documentation and billing activities within the electronic medical record. Provide education regarding medications, treatments, nutrition, and disease management; address barriers to care; support drug assistance programs; coordinate home health services; distribute and track home health orders; enter home health charges for billing; and promote patient understanding of physician instructions and treatment plans. Obtain outside records and diagnostic results for upcoming visits, review charts to ensure readiness and quality standards, participate in staff meetings and educational sessions, collaborate with team members to provide coverage during absences, and perform additional duties as assigned to support clinic and Department of Medicine operations. Other duties as assigned by clinical nurse supervisor or Director of Nursing Qualifications Minimum Qualifications and Skills: Completion of a program of instruction for licensure as a Practical Nurse required. Completion of BLS Class and re-certify every 2 years. Must be a licensed LPN in Virginia. Previous clinical experience preferred. Montreal Cognitive Assessment (MoCA) certified desirable. Preferred Qualifications: Ability to develop and maintain effective relationships with public, partners, customers, and co-workers; while working cooperatively and effectively with others to achieve common goals. Ability to manage time and resources to ensure assignments are completed accurately and deadlines are met. Meets attendance/punctuality requirements. Ability to adapt to changing work environments, work priorities, and organizational needs. Takes independent action to address and resolve problems. Licensed Practical Nurse (LPN)

Overview The Department of Family and Community Medicine at Macon & Joan Brock Virginia Health Sciences at Old Dominion University is seeking an outstanding Licensed Practical Nurse (LPN) to join the team. The Licensed Practical Nurse (LPN) is responsible for overseeing and assisting with the day-to-day patient care including but not limited to taking vitals, administering meds/treatments, assisting physicians, doing wound care, patient education, documentation, coordinating care, focusing on comfort and supporting quality initiatives, in a fast-paced office. LPN will also assist with training new staff and a point of contact for medical assistants. Responsibilities Triage incoming patients by obtaining vital signs, following standing triage orders, reconcile medication list, reconcile allergies, make sure immunizations are up to date and prepare patients for the visit. Completes delegated tasks from the current and prior day i.e. patient messages, prescription refills, lab results, referrals, home health requests, medication prior authorizations, patient forms, and physician follow-up. All tasks must be reviewed, addressed, and marked appropriately before the closure of each business day. Performing point-of-care testing and collection of lab specimens for further processing. Assist physicians with examinations, minor surgeries, diagnostic procedures and treatments. Provide basic patient care and education as directed by provider and standing orders. Maintaining cleanliness in treatment areas and ensuring exam rooms are fully stocked with necessary medical instruments and supplies. Ensure stocked items are not expired. Cleans, sterilizes, and maintains used instrumentation per department protocol. Assists supervisors in the training of new staff members. LPN's are required to fill in for other clinical staff during vacations and illness and perform other duties as assigned by providers, or supervisors at both sites. (Ghent and Portsmouth Family Medicine.) The successful candidate in this role shall respond to questions from patients, family members, faculty/residents, other health care providers in a timely and professional manner. Patient inquiries should be answered on the same day they are received, before leaving for the day. If patient is not home at the first attempt, a notation should be made in the medical record. All patient conversations must be documented in the electronic medical record. Follow EVMS Medical Group protocols related to infection control, environmental safety and patient confidentiality. Complete annual recertification for Blood Borne Pathogens, HIPAA Compliance Training, Respiratory FIT and IT Policy. Maintain current CPR certification and LPN licensure. Qualifications Minimum Qualifications and Skills: Licensed in Virginia or hold a valid multistate compact license issued by applicant's primary state of residence. Completion of BLS training and recertification every 2 years Preferred Qualifications: Previous medical office experience with relevant skills preferred.
