LVN s with Home Health experience will be given preference. The primary purpose of this position is to provide clinical support for the processing of home health orders, labs, and physician communication for home health location. Under the supervision of the Administrator, the LVN Care Coordinator (LVN CC) provides clinical coordination and communication with patients, physicians, and other clinicians in accordance with current standards and regulations so that the highest degree of quality care will be maintained at all times. Completes other tasks and projects as assigned by the Manager when necessary. Job Description : EDUCATION Equivalent experience will be accepted in lieu of the required degree or diploma. Other: Graduate from an accredited vocational school of nursing or equivalent. CERTIFICATION & LICENSURE LVN-Licensed Vocational Nurse PREFERRED EXPERIENCE AS TYPICALLY ACQUIRED IN: 1 year clinical experience required; two years clinical experience strongly preferred 1 year of experience in a home care position strongly preferred Experience in coordinating and processing home care orders strongly desired.
