The incumbent delivers medical care to a wide variety of patients. The incumbent examines and treats patients independently and in autonomous collaboration with other health care professionals. CORE JOB FUNCTIONS Performs and documents complete physical examinations and comprehensive health histories. Functions independently to perform age-appropriate history and physical for patients. Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs. Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities. Implements interventions to support the patient to regain or maintain physiologic stability. Assists with the provision of care in accordance with facility, state, and federal regulations. Monitors the effectiveness of interventions. Facilitates the patient’s transition within and between health care settings, e.g., admitting, transferring, and discharging patients. Analyze and interpret data to determine treatment recommendations and priorities. Screen for nutrition deficiencies and make interventions or refer for a nutrition consultation as appropriate. Screen for pain and understands treatment strategies for pain including pharmacologic and nonpharmacologic approaches. Strategies should reflect an individualized approach based on the individual’s current presentation, medical judgment and the risks and benefits associated with strategies, including potential risk of dependency, addiction, and abuse. Clinical (Both Ambulatory and Inpatient) A. Assesses the physical and psychosocial status of clients by means of interview, health history, physical examination, and diagnostic studies. B. Recognizes deviations from normal in the physical assessment. Works in collaboration with a physician in formulating treatment plans for health problems and follow-up. C. Writes prescriptions for medications and blood products based upon laboratory results, orders and interprets routine diagnostic and follow-up studies, therapeutic measures, and postdischarge care in accordance with written practice protocols. D. Requests written consultation from physicians and other healthcare professionals to ensure appropriate and quality patient care. E. Assesses and follows consult patients when required and independently of physician when needed. F. Interprets and evaluates findings of studies/tests and intervenes accordingly. G. Advises collaborating physician of appropriate information regarding patient care and reports to the physician the actions provided to the patients. H. Performs bone marrow biopsies, lumbar punctures with IT chemo, Omaya reservoir access, PICC line removals and skin biopsies if competency met. R. Act as a preceptor for APP students, fellows and new APP staff as appropriate. S. Ambulatory Specific: 1. Rotates with colleagues taking weekend and holiday call from Sylvester patients. 2. Responds to medical and oncologic emergencies in the CTU and clinics. 3. Communicates with triage nurses regarding patients who need to be added to OCC clinic for symptom management, evaluation, and treatment. T. Inpatient Specific: 1. Interacts with admitting physician, hospitalist, and other multidisciplinary teams. 2. Prioritizes consults to present to attending physician and rounds with attending daily on patients as appropriate. 3. Writes consult inpatient notes, discharge notes and dictates as appropriate. 4. Rotates with colleagues performing Hematology consultations and inpatient care at UMH as per divisional needs. II. Research A. CITI certification within three months of employment B. Facilitates clinical research protocols and coordinates patient care with research staff. 1. Review clinical trial options with patients and families. 2. Interview and examine research subjects. 3. Evaluate research subjects’ appropriateness for study inclusion/enrollment. 4. Follow all regulatory requirements for human subjects’ research compliance and documentation. 5. Monitor, Review study related labs and imaging, and provide ongoing follow up to hematology patients on clinical trials. 6. Triage research patient communication. 7. Report SAEs and AEs to designated research staff. 8. Communicate with the PIs and Sub PIs with questions or concerns regarding holding or continuing investigational or standard of care treatment. 9.
