Completes the medication reconciliation process by documenting the current list of medications (including over the counter and nutritional supplements) taken by patient at each visit. Performs and documents an appropriate comprehensive physical assessment of patients which will include a review of systems, evaluation of manifestations and toxicities related to medical conditions and anticoagulation management. In collaboration with the physician, critically analyzes the assessment data/diagnostic tests and determines a diagnosis. In addition, develops a comprehensive plan of care that includes interventions of treatments to attain outcomes. Evaluates patient progress in attaining expected treatment outcomes. Monitors for signs and symptoms related to disease progression/recurrence. Appropriately manages medical and anticoagulation management emergencies according to protocol/policy. Prescribes and/or orders medications. Obtains appropriate consents for surgery and procedures. Ensures completion of all needed pre-operative tests, forms, and documents.
