With collaboration of the attending physician, writes antibiotic and/or oxygen orders needed for post discharge. Performs peer to peer appeals on post acute denials. Acts as back up for the Physician Advisor handling peer to peer appeals and written appeals when necessary. Performs secondary clinical assessment and documentation to support inpatient admissions as well as to support subacute, long term care(trach weaning necessity/settings) post discharge. Assists denials staff with open communication and feedback to medical staff in regards to denials due to level of care and insufficient documentation. Supports care coordinators to provide real time feedback to providers for improved documentation that will defend the length of stay. Works collaboratively with care coordination manager in process improvement in regards to length of stay, denials and readmissions. Supports care coordination department with difficult discharges and/or refusal to leave facility to a more appropriate level of care. Qualifications Bachelor's degree in Nursing required. Current NJ APN license required. Previous healthcare experience required. Previous experience in an inpatient hospital setting preferred. Experience with EPIC preferred. Responsibilities: Perform thorough psychiatric evaluations and assessments for elderly patients in nursing homes. Develop personalized treatment plans based on patients' mental health needs and diagnoses. Administer medication management and adjust treatment plans when required. Collaborate with interdisciplinary teams, including physicians, psychologists, and social workers, to ensure holistic patient care. Provide counseling and psychotherapy services to patients and their families. Educate patients and their families on mental health conditions, treatment options, and self-care strategies. Maintain accurate and up-to-date medical records in accordance with organizational and regulatory standards. Actively participate in clinical meetings, case conferences, and quality improvement initiatives. Requirements: Hold a valid Nurse Practitioner license in the state of New Jersey with a specialization in Psychiatry (PMHNP). Demonstrate the ability to work independently and effectively manage a caseload of patients.
