In this role, you will use phone and video-based care to provide high-quality, comprehensive follow-up care to patients who are post-hospital discharge. Here's what you'll do: Cover our care needs for our Transitions of Care program. Day to day work includes telephonic outreach (“cold calls”) and scheduled video/phone visits for patients who have recently been hospitalized and discharged. Visits focus on: Medication reconciliation, symptom screening, and ordering of any needed follow-up labs, medications or supplies. You will also navigate conversations with patients to build trust, explain Galileo's care model and connect them to services. Coordinate care across settings — hospital and ED transitions, pharmacies, and community providers — to keep patients connected and moving forward. We work in 4-hour shifts (mornings, afternoons, evenings, overnight on weekdays and weekends). At 24+ hours you will be considered full-time and benefits eligible. Providers are expected to work weekends on a monthly basis, specific schedule will be influenced by total hours committed and patient care demands. We are currently looking for a Nurse Practitioner with an active license in PA who is available to work 24-36 hours/week. Required Qualifications: Master's Degree from an accredited university in a Physician Assistant Program or Master's Degree from an accredited university in a Nurse Practitioner program Valid and unrestricted professional PA or NP license in the state which you reside Ability to apply for 10+ additional assigned state licensures within 2 weeks of hire date Collaborative physician agreement if applicable based on state scope of practice regulations 1+ years as a PA or NP in a clinical environment including oversight of ancillary and clinical staff and caring for adults Experience conducting assessments on wide variety of acute/chronic conditions Proven experience utilizing electronic health record (EHR) platforms to document, review, and manage patient information Proven excellent interpersonal and problem-solving skills Ability to work one or more of the listed schedule requirements including one weekend rotation per month Preferred Qualifications: Multiple licensures preferably in one or more of the following states with ability to obtain multiple state licenses per regional assignment: KS, OR, AR, AK, IA, ME, KY, CT, NH, WV, DE, MT, ID, SD, NM, RI, ND, VT, WY, WY, NE 2+ years clinical experience in a health care delivery system VA and/or military experience Experience working in a Virtual setting Experience working in a call center environment, handling a high volume of calls and performing repetitive tasks while maintaining quality and productivity standards Proven leadership experience including the ability to supervise, coach and develop staff using excellent interpersonal and communication skills Familiarity with government forms and documentation Excellent computer skills in Microsoft Office and database experience Proven excellent written and verbal communication skills Proven excellent social and therapeutic communications skills *All employees working remotely will be required to adhere to UnitedHealth Group's Telecommuter Policy Compensation for this specialty generally ranges from $113,000 - $173,500. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
