Cultivates and maintains effective interaction/communication with members of medical staff, nursing staff, social workers, and others to drive care coordination process and facilitate continuity of patient care. Communicates with all members of multidisciplinary team to facilitate care coordination process for assigned workload. Communicates with home health agencies, third-party payors, and other community resources as needed to coordinate discharge needs. Facilitates and provides ongoing communication with patient/family and interdisciplinary staff to identify and resolve potential barriers. Facilitates care conferences, interdisciplinary rounds, and other meetings. Participates in focused system initiatives and facilitates clinical practice guidelines at the patient level. Performs outpatient and clinic care coordination and monitors care as patients transition between outpatient and inpatient services where appropriate. Job Description The PICU is seeking a Registered Nurse to join our team in Wilmington, DE. This is a 36hrs/wk, rotating shift position. The Pediatric Intensive Care Unit of Nemours Children's Health, Delaware is a 24-bed unit in a Level 1 Trauma Center. Patients requiring care and surveillance in the PICU will have conditions and diagnoses which may include, but are not limited to, those producing: acute and reversible loss of consciousness, respiratory failure, compromised air exchange, cardiovascular dysfunction, hepatic failure, hemodynamic instability, renal dysfunction, central nervous system injury or dysfunction, acute trauma and organ failure requiring transplantation. Treatments or interventions performed in the PICU include measures utilizing advanced life support technology. These include, but are not limited to cardiopulmonary resuscitation, tracheal intubation, ECMO, pharmacologic support of circulation, mechanical ventilation, point of care monitoring, CRRT, thoracentesis, and hemodynamic, intracranial pressure, intravascular monitoring. Each patient room has electrocardiographic, pulse oximetry, respiratory, hemodynamic and intracranial pressure monitoring capability, as well as central EKG and respiratory monitoring. Portable monitoring during patient transport is provided. Special equipment includes radiant warmer beds, heating/cooling blanket systems, Mapleson manual ventilation system, end tidal CO2 monitors, ventilators, hemodynamic monitoring systems, intracranial pressure monitoring system, ventriculostomy systems, infusion pumps, blood warmer, continuous renal replacement systems, and peripheral nerve stimulators. Case Manager, RN - Care Coordination

$51.60 - $91.30 / hour
Case Manager, RN - Care Coordination 1.0 FTE, 40hrs per week, 8hr shifts. Start time 630AM, 7AM or 730AM. Has responsibility, in collaboration with the health care team and family, for the coordination of care for specific patient populations from admission through discharge. This includes advocacy, accountability for the achievement of identified outcomes in a timely manner, promotion of collaborative practice, appropriate use of resources, facilitation of timely discharge, coordination of discharge planning, resource acquisition, and patient/family education. This is accomplished through initial and ongoing assessment, effective participation in the care planning process, and collaboration with the team and community providers. Supports staff/physician education related to the care coordination process. Required Education and Experience BSN degree or equivalent combination of education and experience. Minimum of three(3) years of recent clinical experience. Ability to communicate in a second language may be required for specific departments. Required Credentials Current Washington State Nursing License. At time of hire: Current Basic Life Support (BLS) for Healthcare Providers is required for all positions. Pediatric Emergency Assessment, Recognition and Stabilization (PEARS), and Pediatric Advanced Life Support (PALS) will be accepted in lieu of BLS for Healthcare Providers. As a condition of employment: Current BLS for Health Care Providers is to be maintained at all times by all nurses. Note: current PALS and/or PEARS will be accepted in lieu of BLS, and must be maintained as current at all times. ACLS will not be accepted as a substitute to the requirements listed above. PALS certification may be required for specific departments. Case Managers working in Child Psych Consultation are required to obtain Safety Care Certification (internal training) within 3 months of hire and annually thereafter. Preferred BSN/MS/MSN degree. Minimum of five (5) years of recent clinical experience. Pediatric clinical expertise in area of responsibility. Experience in continuity of care/discharge planning. Compensation Range $51.60 - $91.30 per hour Salary Information This compensation range was calculated based on full-time employment (2080 hours worked per calendar year). Offers are determined by multiple factors including equity, skills, experience, and expertise, and may vary within the range provided.
