Position Summary The Advanced Practice Registered Nurse collaborates with physicians and interdisciplinary teams to provide comprehensive palliative and hospice care in various settings. The APRN delivers expert clinical evaluation, treatment, and management of patients with serious or terminal illnesses. Responsibilities include assessing patient needs, facilitating goals of care discussions, prescribing treatments, and supporting transitions of care. The APRN plays an integral role in educating staff, families, and the broader medical community about palliative and hospice care. PRN as needed for Day Shift Essential Functions Provide medical expertise in pain and symptom management, including terminal prognosis evaluation. Act as attending provider in hospice contract beds in collaboration with the hospice physician. Coordinate admissions and discharges for hospice contract beds, including review of terminal diagnoses. Assess patient conditions, develop care plans, implement interventions, and evaluate outcomes. Conduct patient and family assessments, ensuring readiness for transition to hospice or home settings. Perform physical exams, diagnostic testing, treatment, follow-up, and consultations. Prescribe and administer medications and other therapeutic interventions. Collaborate and consult with hospice/palliative physicians, attending physicians, and interdisciplinary team. Support patients and families in clarifying care goals and end-of-life decisions. Maintain accurate and complete medical records to ensure compliance and reimbursement. Ensure informed consent documentation, including CPR and DNR discussions. Participate in interdisciplinary team meetings and on-call rotations. Educate internal staff and community members on palliative and hospice care. Coordinate inpatient APRN assignments and support departmental functions . Qualifications Knowledge and Critical Skills Expertise in patient charting, history taking, and documentation practices. Knowledge of health education methods, clinical operations, and medical procedures. Proficient in diagnosis, treatment planning, and symptom management.
