IntellaTriage Nurses enjoy the benefits of working from home, providing care, without the stress and expense of commuting each day! **MUST have or be willing to obtain a Compact RN license **MUST live in/work from a Compact US state **Minimum of 2 years as a Nurse **Minimum of 1 year of experience in a fast-paced environment (i.e. ED, Critical Care, Surgical Services, Med/Surg, etc.) **Must have high speed internet **Must be tech savvy, enjoy a fast-paced environment, and have no concerns typing **Must be available to work 2 out of every 3 weekends Our Nurse Advice Line Nurses: Have proven experience in a fast-paced, critical-thinking environment; ED, Critical Care, Surgical Services, etc. Work a minimum of 2-3 shift sper weekday (Mon-Fri). Preferred scheduling for their weekday requirements. Work 2 weekends or every 3 weeks, both Saturday and Sunday on those weekends with rotating times. (For example: weekend 1: work, weekend 2: work, weekend 3: off) Train for 3 weeks. (Week 1 Mon- Thurs 9a-6p CST) Must have a compact license and live in that compact state. (no states with pending legislation or future implementation dates are considered current compacts) What is important to know? We are growing and excited to be able to support our clients’ nursing staffs in the field who need time to focus on work-life balance, as well, while being able to trust that we are there to support them and their patients during nights and weekends! Patient care is #1. We do not have call quotas. We employ the best nurses to provide the best care. When our patients or their families reach our triage line, they immediately speak directly with a nurse. We do not have PRN positions. We are super busy. If you like fast-paced roles, keep reading… Any nurse may pick up additional shifts, if shifts are available for the clients’ they are trained to support. We will provide you with a laptop and headset. Nurses are required to provide their own high-speed internet (only fiberoptic or coaxial cable internet is compatible with our remote call center technology). Our laptop is required to be directly connected to your modem. Working through Wi-Fi is not compatible with our systems. The calls will drop. It is essential to have a home office or quiet space free from noise or distractions in your home (Privacy/HIPAA compliant space is required). Training is provided remotely and is paid; no travel is required in this role. MUST be able to follow instructions (verbal and written) and be comfortable with technology (tech savvy). Must remain in good standing and ensure their home state license remains active. IntellaTriage will cover the cost of non-compact state licensure based on the client(s) that are assigned for support. All nurses must have a compact license and reside/work in that state. Shift prep is a minimum clock-in of 30 minutes prior to taking calls; this is paid time to prepare. Once calls roll to the next team/shift, our nurses remain clocked in and complete any remaining charting before leaving for the day. This may take 30 minutes or this may take 2 hours. It depends on the pace of the calls received during that shift and the pace of your ability to quickly navigate technology and type. Sound exciting to you? Must be comfortable with technology and accessing multiple applications remotely to perform documentation during calls. Ability and comfort typing. Fluency in English is required, additional languages are a bonus. Must physically reside in the U.S. and be legally eligible to work for any employer. Must be able to complete the 3-week orientation and training (Schedule listed in this posting). Must be available to work Saturday & Sunday on your team's required weekends; 2 of every 3 weekends. Holidays as they are required (rotation). Able to handle stress and multitask when calls are coming in (minimum of 5+ calls per hour on weekdays, and much higher on weekends). Able to communicate with patients and families with empathy while also maintaining adherence to client protocols. Must maintain CEU’s as designated by the states you are answering calls in. Must attend any in-services, additional training on an as needed basis. Able to pass background check and nurse licensing check. All Remote Nurse Advice Line RN's, once trained to their originally assigned team are at $25/hourly. All part-time nurses accumulate PTO, based on the number of hours they work (per year). All part-time nurses receive an additional 3-paid-sick-days per year. All part-time nurses are eligible to participate in our 401(k) plan.

Clinical Guide Part A: Utilization Management Nurse

Job Description Schedule: The weekly schedule choice is either Monday - Friday 10am-7pm EST OR Tuesday - Saturday 9am-6pm EST A bit about this role: The Clinical Guide Part A will be part of the Utilization Management team, responsible for inpatient, behavioral health, and/or post-acute authorization review in alignment with CMS and Medicare Advantage regulations. Reviews medical records to evaluate the medical necessity and appropriateness of requested inpatient and/or post-acute services in accordance with established clinical criteria and CMS guidelines. Your Responsibilities and Impact will include: Review Medical Records: Conduct prospective (pre-service), concurrent, and retrospective utilization review to evaluate medical necessity, appropriate level of care (Inpatient vs. Observation), and post-acute services in accordance with established clinical criteria and CMS guidelines. Evaluate Treatment Plans: Assess the appropriateness, timing, and setting of requested services, ensuring alignment with medical necessity criteria and Medicare Advantage requirements. Recommend alternative levels of care when clinically appropriate. Inpatient & Behavioral Health Review: Perform initial, concurrent, and discharge reviews for inpatient and behavioral health admissions. Ensure admission status accuracy and regulatory compliance with CMS timeliness (TAT) standards. Post-Acute Review: Conduct initial authorization and concurrent review for post-acute services (SNF, LTACH, ARU, Home Health), evaluating ongoing medical necessity and appropriate length of stay. Issue NOMNC when coverage criteria are no longer met. Medical Director Collaboration: Refer cases that do not meet criteria to the Medical Director for secondary review and final determination. Prepare clinical summaries and coordinate peer-to-peer (P2P) discussions. Manage authorization reopen requests as appropriate. Resource Stewardship: Monitor utilization of inpatient and post-acute services to promote appropriate resource use while maintaining high-quality, member-centered care. Regulatory & Documentation Compliance: Maintain accurate, defensible documentation of all determinations. Ensure adherence to CMS regulations, Medicare Advantage requirements, and internal compliance standards. Required skills and experience: Unrestricted RN license with a minimum of 4 years of clinical experience. Minimum 3 years of Utilization Management or Inpatient UR experience within a health plan or hospital setting. Strong knowledge of CMS regulations and Medicare Advantage requirements. Experience preparing cases for Medical Director review Able to work in a fast paced environment that is constantly evolving. In this work-from-home role, you’ll deliver after-hours care virtually (primarily by phone) to aging residents across multiple states. This position offers both autonomy and purpose—you’ll be the trusted voice and clinical partner helping residents and facility staff during critical times, making an immediate impact in the lives of older adults. In this position the provider must be comfortable managing high call volumes and performing at least 30% telehealth visits, including evaluation of acute changes, falls, and controlled substance visits. Providers must be able to manage multiple calls independently while providing care across several states. Essential Duties & Responsibilities Serve as the first line of support for residents and facility staff after-hours, providing direction and medical care over the phone. Use Curana’s telephonic platform to take and place calls, coordinating care between facilities, hospitals, and clinics. Deliver high-quality, cost-effective care to patients—addressing acute, chronic, and behavioral health needs in collaboration with physicians and specialty providers. Perform comprehensive assessments and document encounters accurately and thoroughly in the EMR, ensuring compliance with CMS requirements. Apply Curana’s clinical protocols and practice guidelines to support safe, effective treatment in place whenever possible. Participate in mandatory education and training to stay current with standards of care. Scheduling & Hours: While shift times can vary, we provide coverage to skilled nursing and senior living facilities on weeknights from 5pm- 8am local time, continuous coverage from Friday at 5pm to Monday at 8am. Holiday coverage is also provided beginning at 5pm of the end of the last business day to 8am of the resumption of business hours. We deliver 24/7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans to break down barriers to quality care. The primary location for this position is remote (CST required). Expected schedule will be 3x12s (Monday-Wednesday or Wednesday-Friday) 7:00am-7:00pm CST. What You’ll Do As a Pediatric Registered Nurse at Imagine Pediatrics, you are the primary point of contact for our families as you work to deeply know our patients through frequent virtual touchpoints and are the first line of defense when our patients are having a clinical problem. You leverage an integrated technology platform and are complimented by an entire interdisciplinary team including MDs, APPs, social workers, navigators, pharmacists, and dietitians. In this role, you will: Provide professional and friendly proactive care and triage for clinical issues. Embed a family centered care philosophy in care delivery. Demonstrate cultural competence and sensitivity as ability to work with culturally diverse populations and seek out additional resources when needed. Transition of care for ED/IP/UC care coordination with clinical providers following discharge. Perform a comprehensive assessment of a patient’s clinical, psychosocial, discharge planning and financial needs. Establishes clinical milestones and goals related to these issues. Establish rapport and a relationship with the patient and family in order to understand their needs and expectations and to assist them in setting realistic and mutual goals. Integrate an awareness of cultural factors in the patient/family interview process and elicit clinically relevant cultural information. Establish, in conjunction with the physician, the patient and interdisciplinary team, a comprehensive plan of care to appropriately address clinical milestones. Communicate plan of care, including changes and issues related to plan of care to patient/family, physicians and other members of the healthcare team. Gather sufficient information from all relevant sources to determine the effectiveness of the plan of care to assure it is done in an accurate, safe, timely and cost-effective manner. Document all care management assessments and interventions. Refer to Social Worker or Behavioral Health for complex psychosocial and discharge planning issues (per criteria) and ensures appropriate follow-up. Consults with other members of the interdisciplinary team (dietary, pharmacy, etc.) to provide safe discharge as appropriate.
