DUTIES: • Schedule and conduct face to face comprehensive clinical assessments and provide care for potential clients according to state guidelines, and policies, procedures, and protocols as described by Community Care Home Care. • Conducts comprehensive assessment of all aspects of potential new members condition - clinical, social, and environmental to determine the level of services needed to appropriately keep member safe in the community • Gathers clinical information from various sources to address all conditions, and follows appropriate guideline, policy, procedure and clinical judgment in completing assessment requirements • Communicates with consumers, families, providers, and other parties as needed to complete accurate comprehensive assessment • Understands the needs of vulnerable populations who have chronic or complex bio-psychosocial needs • Conducts all assessment functions in compliance with regulatory and company policies and procedures REQUIREMENTS: • Must hold valid Registered Nurse (RN) license. • Proficiency with computers is required • Effective communications, organizational, and interpersonal skills required • Ability to work independently is required • Effective computer and English language skills required • MLTC/ Managed Medicaid experience preferred **Community Care Home Health Services is committed to ensuring the safety and wellbeing of our patients and team members, so we may continue serving our communities by delivering exceptional care. Position Overview: The Night shift Supervisor is responsible for directing the operation of a nursing unit in a long-term community. Key Responsibilities: Perform general management functions such as hiring, disciplining, and evaluating care team members. Plan and facilitate meetings and committees to address resident care issues/concerns for the unit. Manage area of responsibility with the goal of achieving and maintaining the highest quality of care possible. Participate in developing, implementing, and evaluating programs that promote the recruitment, retention, development, and continuing education of nursing staff members. Contact physicians as needed/necessary and ensure physician interventions are timely and appropriate. Review staffing patterns and census of nursing units and reassign personnel when necessary to ensure staffing meets resident needs and budgetary controls. Monitor the clinical operations of the unit and resident’s conditions and ensure that appropriate and quality care is administered. Obtain medications, supplies and medical records needed to provide safe, efficient, and therapeutic are to residents on a continuing basis. Assist in the orientation of new personnel, monitoring their skills and guides and observe care team members that may need assistance with procedures. Qualifications: Graduate of accredited school of nursing. Licensed as an RN in the state employed; license must be active, valid, and in good standing. Valid CPR teaching certificate preferred, but not required. Must hold an active CPR license or the ability to obtain within the first 30-days of employment. CPR license must be kept active and current throughout employment. Three years of nursing experience, of which one year in a long-term care environment.
