Transplant Coordinator (RN) - VAD

A dynamic professional clinician focused on population management with demonstrated capability to provide relationship-based care within a collaborative multidisciplinary team. This registered nurse utilizes advanced critical thinking to develop, implement, and evaluate an individualized plan of care that educates, empowers and optimizes patient outcomes throughout all transitions within the continuum of care. In addition, this position requires clinician provide afterhours/weekend coverage to serve as primary point of contact for pre and post-transplant patients and internal and referring providers. Business and after hours care coordinator responsibilities include telephone assessment using strong clinical knowledge base, complex care coordination and treatment based on comprehensive assessment and strong interpersonal communication skills, calling in patients for transplant, and ensuring all multidisciplinary members of the transplant team are kept informed of clinical decisions and patient response to interventions in a timely manner. Transplant coordinators must be knowledgeable of the principles of ethical practice and have resources available to evaluate the merits, risks, and social concerns of transplantation. Promotes the advancement of the transplant profession through dissemination of information, policy advocacy, innovation, and influence of decision-making bodies. Participates in Transplant Center Quality Assurance & Performance Improvement (QAPI) program meetings, projects and events. Innovation: is demonstrated by application of technologies that support patient care, actively seeking to implement evidence based practice and new knowledge generated by nursing research. Participates in the research and publishing of new initiatives and groundbreaking Transplant Center treatment interventions. In addition to the above job responsibilities, other duties may be assigned. MINIMUM REQUIREMENTS Education: Bachelor of Science in Nursing from an accredited nursing program required. Experience: 2 years of relevant experience. Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. Registered Nurse (RN) Imaging Job Responsibilities Performs comprehensive patient health assessments by interpreting multiple data sets and synthesizing their interrelationships Formulates individualized plans of care based on assessment findings and interpreting trends in patient populations to achieve best practice Accesses resources from all disciplines to achieve desired patient outcomes and coordinates healthcare and alternate services for patient/family transition planning Makes clinical decisions based on experience, patient population data and intuition while promoting the development of clinical decision making of all team members Assists staff to assess patients' learning needs and outcomes measurement of patient education Provides direction and guidance to others regarding practice while serving as a clinical resource, preceptor, and mentor and delegates patient care activities and coordinates unit activities as needed Utilizes standards, guidelines and pathways for care delivery and ensures that the process for delivery of care is synthesized from direct patient/family communication, technology and other healthcare team member collaboration Demonstrates evidence of self-development by obtaining a minimum of six formal contact hours and maintains own Team Member Continuing Education Record Evaluates clinical and quality outcomes and contributes to performance improvement initiatives May perform additional duties as assigned Minimum Qualifications Education: BSN or AD. If RN has an AD, within 2 years from date of hire, they must meet with their nurse leader and conduct the following: 1.) Identify which accredited school they plan to attend 2.) Provide a written plan with anticipated BSN completion date 3.) Submit a review of transcripts from the school indicating the required pre-requisites and timeline for taking the courses 4.) Complete BSN within 5 years of start date Experience: 1 year of experience as a Registered Nurse, for Critical Care units, 1 year of experience as a Registered Nurse in a critical care setting or completion of a critical care fellowship. Certification: Basic Life Support from the American Heart Association; Licensed or eligible for licensure in the Commonwealth of Virginia as a Registered Nurse or an active multi-state Registered Nurse license Preferred Qualifications Experience: 2 + years of RN experience in Imaging, ICU, ED, Cardiac, PACU, or another critical care environment. Confidence in administering medications independently and reading/interpreting ECGs independently. Certification: ACLS from the American Heart Association upon start Other: Flexible and adaptable with scheduling; Comfortable working a variety of shift lengths and days based on departmental needs; Willing to participate in the rotating Saturday on-call schedule.
