Responsibilities: Reviews daily reports of patients readmitted within 30 days of discharge and identifies the primary cause of readmission. Maintains a readmission database and provides reports as needed to facilitate readmission mitigation and reduction. Evaluates the current census to identify patients at a high risk for 30 day readmission and rounds on patients to provide and evaluate appropriate prevention measures have been implemented Develops readmission mitigation strategies by leading a diverse team of internal professionals that may include a Nurse Care Manager, Social Worker, House Supervisor, Respiratory Navigator, Clinical Nurse Specialist, Physical therapist, and Post-acute Care representative. Members of the team may be identified on an as needed basis determined by the specific reasons for readmissions. Interacts collaboratively with all departments to foster development of readmission prevention strategies. Acts as a department resource for high complexity readmission issues. Performs concurrent chart audits for readmitted patients to identify opportunities to prevent additional readmissions. Preforms retrospective chart audits to identify causes of readmission and determine potentially avoidable causes. Interacts with physicians and other healthcare team members when indicated to facilitate appropriate patient transition of care. Maintains current knowledge in the field by attending seminars, continuing education courses and workshops. Collaborates with medical providers, patient care staff and management in the planning and implementation of patient and staff education. Assists with the development of care plans that prescribe specific interventions or life style modifications to attain expected outcomes. Within the Cath Lab, this may include structural heart procedures and hemodynamics; within Electrophysiology (EP) Lab, RN’s perform device and electrophysiology procedures (EP), and operate 3D mapping systems. Will be required to work varied hours, shift lengths, and respond to an on-call pager, which may include days, evenings, nights, holidays, and weekends. Responsibilities: Prepares the patient care and/or procedure area prior to the arrival of the first patient or beginning of first procedure to provide for safe, efficient delivery of care. Receives and reviews charts for scheduled patients as soon as they are available for the next day. Assures that proper documentation, lab results, and consent is present in the chart prior to the procedure. Communicates abnormal lab results or other concerns to the physician/Nurse Practitioner. Prepares the patient for the procedure in a timely and efficient manner. Provides nursing care, including complete systems assessment, and monitoring for the immediate pre and post procedural cardiovascular and electrophysiology patient. Accurately records/documents procedure log in MacLab/CardioLab documentation system in real time.
