Department: Hospice Daily Work Times: Variable Shift: variable Scheduled Bi-Weekly Hours: variable PRN/Casual Position Summary: Implements the agency program of full time professional nursing service to patients in their homes under the supervision of the Clinical Services Supervisor in accordance with agency standards and policies. Essential Functions and Responsibilities: Initiates and provides medically prescribed skilled nursing care and/or health counseling that contributes to the treatment of the medical condition, achievement of the rehabilitation potential, and/or reduction of pain and other symptoms. Makes the initial visit to assess the patient’s physical and emotional status, needs, and resources through observation, interview and analysis of records. Prepares the nursing plan of care, in accordance with the physician’s plan of treatment, based on problems identified in the nursing assessment and provides patient teaching using the Care Plan/Standard of Care. Interprets to the patient and family the implications of the diagnosis and nature of the treatment, consistent with the Physician’s instruction. Provides emotional support and encouragement to help the patient and family accept and adjust to the effects of disease, disability, or dying. Supervises and evaluates patient self care or care given by others. Notifies physician and Clinical Services Supervisor of changes in patient condition. Required: Graduate of an accredited school of nursing. Preference will be given to applicants with educational preparation appropriate to the immediate or long-range needs of the agency. Current license to practice as a Registered Nurse in the State of Michigan. Minimum of one to two years of med/surg, critical care or demonstrated experience in a specialty area. Current Michigan driver’s license and proof of valid automobile insurance. Preferred: Two years hospital experience. Experience in a certified home health agency. Responsibilities The Registered Nurse is responsible for the delivery of safe patient care utilizing the nursing process of assessment, planning, implementation and evaluation. Provides direct patient care within the scope of practice. The Registered Nurse also directs and guides patient/family teaching and activities of other nursing personnel while maintaining standards of professional nursing care in Telemetry. The Registered Nurse is directly responsible and accountable for the care given to his/her patients in the Emergency Department. He / She communicates with the physician about the changes in the patient�s clinical condition including results of the diagnostic studies and symptomatology. Is capable of responding quickly and accurately to changes in the condition and/or response to treatment. Is knowledgeable about EMTALA regulations and reporting of child/adult abuse to appropriate agencies and Hospital Social Worker. Maintains patient privacy and confidentiality. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. Current and valid state RN License.2. Current BLS certificate upon hire and maintain current.3. Current ACLS certificate 30 days upon hire and maintain current. 4. Current PALS certificate and/or ENPC 30 days upon hire and maintain current. 5. Basic Arrythmia Interpretation within 30 days of hire.6. Minimum of one year experience as a staff nurse RN in an acute care hospital setting; E.R. setting preferred.7. Certified Emergency Nurse (CEN) preferred.8. Bachelor of Science in Nursing (BSN) preferred. Facility Specific Requirements (facility may require items listed below): 1. In Paramedic Base Station ED: MICN within 12 months of hire and maintain current. 2. EDAP certified facilities: Completion of emergency pediatric course 1 and 2 within 6 months of hire.3.
