Enforces patient rights by assuring patient access to Advanced Directive and Health Care Proxy (HCP) information. Assists in developing system to streamline processes, minimize resource utilization and maximize efficiency. Supervisor is readily available to those with questions, ideas, and or concerns. Communicates and coordinates with nursing units and patient placement coordinator to expedite Emergency Department (ED), PACU admissions and in-house transfers. Seeks solutions to problems identified. Considers financial impact when making decisions. Ensures Purposeful rounding is completed and communicates any concerns with charge nurse/ANM. Job Details Our Hub Manager (RN) will: Collaborate and coordinate with multidisciplinary teams to facilitate a safe quality experience for patients through the continuum of care. Evaluate bed requests and assign beds on a priority basis to facilitate flow. Efficiently place each patient and verifies he/she is in the appropriate level of care and correct patient status/type. Serve as an expert resource in patient flow, status and progression and provides ongoing education to staff, physicians, patients, and their significant others. Provide centralized one-call service for direct admits and facility transfers. Evaluate appropriateness of admission (patient status, level of care) for direct admits and facility transfers. Liaison between all patient care services to ensure appropriate bed is assigned, patient care needs are met and treatment plans are executed in the appropriate level of the continuum. Recognize and take appropriate action in situations that require immediate intervention. Manage Hub/facility staffing for assigned and next shift, if applicable. Proactively identify and remove barriers that may impede department or system-wide patient flow.
