Assistant Nurse Manager - Adult Acute Internal Medicine (4/5 South)

The Adult Acute Internal Medicine Units are both 13-bed general medicine inpatient units that serves a variety of patients. The unit is designed to provide high quality, personalized care for patients. We are pleased that we are able to provide our patients with private rooms, granting them the quite atmosphere to rest and heal. We Treat Diseases and Disorders from the Following Body Systems, Including but Not Limited to: Circulatory Dermatology Endocrine Immune Gastrointestinal Neurological Pulmonary Renal We use a multidisciplinary approach to patient care that is patient centered and encourages teamwork between nursing staff, physicians, therapy teams, social work, care coordination, and pharmacists. We rely on teamwork to provide high quality and compassionate care as we provide patients the necessary care and education for them to make a safe transition home following their short stay in the hospital. Staff Nurse, Adult Electrophysiology Lab – UI Heart and Vascular Center

(Current UI employees are not eligible for incentive) The UI Heart and Vascular Center is seeking a full-time (100%) Staff Nurse to provide direct nursing care to patients undergoing Electrophysiology (EP) procedures and ensure open and effective channels of communication with patients, family/caregivers, and all health care team members involved in the pre/intra/post-care of EP patients. Procedures include (but are not limited to): RF ablations, cryoablations, system extraction, and cardiac device implants. We are the best heart hospital in Iowa UI Heart & Vascular Center is recognized by U.S. News & World Report as the best hospital in the state for cardiology and heart surgery patient care. The role of the Nurse Navigator is to facilitate the progression of care across the continuum for the hospitalized patient through collaboration with the interdisciplinary team which includes but is not limited to patient care staff, providers, patient support, family and community resources. The Nurse Navigator ensures appropriate transitions of care to enable quality outcomes. This position is responsible for ensuring the appropriate utilization of clinical resources with the goal of timely and safe discharges while maintaining healthcare industry compliance. Collaborate with the patient’s provider and other healthcare team members in managing the patient’s length of stay and determining the appropriate level of care for transition planning. Assist in discharge planning (referrals) for individuals with continuum of care needs (Post-acute facilities, Home Care, Hospice, etc.). Work collaboratively with Social Work or Care Coordination Team Member to manage social issues that impact the ' transition planning needs, and refer cases of suspected/actual abuse, domestic violence, or neglect to appropriate agencies". Navigate the care delivery system while collaborating with the physician and other clinical departments by ensuring that tests, treatments, consults, and procedures are appropriately indicated and performed timely. Articulate the plan of care and communicate this plan to other care team members and patient/caregiver thereby enhancing patient and staff satisfaction. Intervene to maintain care progression when a deviation in the plan occurs.
