Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care. Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care. Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.). Provides appropriate support at time of death and perform bereavement assessment. What is Required? Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing Current State License as a Registered Nurse RN 1 year of experience in a clinical care setting Experience in a hospice or home health environment is preferred 50% travel required Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Valid driver’s license and insurance and reliable transportation to perform job tasks You will report to the Clinical Team Manager. Provides those services required substantial and specialized nursing skills under the direction of a Registered Nurse or physician. Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions. Documents medication regimen and updates medication profile according to established procedures. Treats patients and caregivers in the highest and most effective manner. Implement all available actions to prevent avoidable hospitalizations and ER visits. Provides positive, supportive communication to physicians, patients, families, visitors, and agency personnel. What is Required? Graduate of an accredited Certificate, Diploma, or Associate School of Nursing Current State License as a Licensed Vocational / Practical Nurse 1 year of experience in a clinical care setting Experience in a hospice or home health environment is preferred 50% travel required Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs. Master’s degree in nursing and certification through a national Nurse Practitioner Licensing board. 1+ years of clinical nursing experience 1+ year post-graduate clinical nursing experience in the hospice and palliative care field preferred Prior experience in Family Medicine, Geriatrics and psychiatry preferred. Makes the initial evaluation and re-evaluates the patient’s nursing needs during each visit. Communicates significant findings, problems, and changes in health conditions, environment, or unsafe facility conditions to the clinical supervisor, the physician and/or other personnel involved with patient care. Counsels, instructs, and implements the plan for patient safety, utilizing patient information, family, and community resources. Participates in the implementation of the plan of care of an assigned case load to ensure quality, continuity of care, and achievement of patient outcomes. Identifies deviations from normal Nursing and normal patient condition status. Initiates appropriate actions based on the deviation. Verifies the Plan of Care prior to each visit and provides care according to physician’s orders, assessment data, and established standards and guidelines. Performs other duties/projects as assigned. Makes the initial evaluation and re-evaluates the patient’s nursing needs during each visit. Makes follow up visits according to patient, family, and facility needs. Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care. Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members. What is Required? Current, unrestricted state RN license. 1+ year experience in a clinical care setting as a nurse. Associates Degree or Bachelor’s Degree in Nursing is preferred. Previous Home Health or Hospice Experience is preferred. Previous experience with HomeCare HomeBase is preferred. Able and willing to travel within the branch/office coverage area. Must be able and willing to travel 50%. Ability to sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs. Valid driver’s license and insurance and reliable transportation to perform job tasks.
