Patient Flow Coordinator - Registered Nurse

What We Offer Position: RN, Patient Flow Coordinator Location: Remote Schedule : PRN, must be able to work 20 - 40 hours for 6 weeks of orientation Department : Behavioral Health Bed Management A minimum of 2 years of in-patient behavioral health RN experience required As an RN Patient Flow Coordinator for Behavioral Health , you’ll play a vital role in ensuring patients receive timely access to the right level of care in the right setting. You’ll serve as a key liaison between Nursing, Patient Access, and departments throughout the hospital to promote safe, efficient, and coordinated patient placement. In this role, you will: Coordinate timely patient placement by collaborating with Nursing, Patient Access, and other hospital departments to ensure patients are accurately assigned to the appropriate bed or unit. Evaluate patient needs and work closely with clinical teams to determine the most appropriate placement based on the patient’s clinical condition, behavioral health needs, and available services. Facilitate efficient patient flow throughout the facility by identifying barriers to placement and coordinating solutions to minimize delays. Serve as a clinical resource and liaison between Behavioral Health, Nursing, Patient Access, and other departments to support seamless transitions of care. Collaborate across disciplines to address placement challenges and resolve issues that may impact patient throughout and the patient experience. Promote safe and appropriate patient placement by balancing clinical needs, unit capabilities, staffing considerations, and available resources. Support a coordinated patient experience by helping ensure patients move efficiently through the hospital and into the appropriate level of care. This is an excellent opportunity for an experienced RN who enjoys collaboration, problem-solving, care coordination, and making a meaningful impact on patient flow and access to behavioral health se rvices. What You'll Do Education: 4 Year / Bachelors Degree, preferred. BSN preferred. Experience: 2 years' experience in in-patient Behavioral Health, required.
