The NICU Transport Nurse acts as a resource hospital wide for patient care standards and pre-code recognition. As a recognized resource, the job assists in establishing and maintaining a high quality standard of care for the neonatal and pediatric transport population under the age of two months. Requires a Bachelors of Science Degree in Nursing. An Associate or diploma prepared nurse will be considered, with the expectation they will be enrolled in a BSN completion program with the understanding that degree completion would occur within three years. Current Wisconsin State License (Compact License) as a Registered Nurse and must pass pre-placement physical exam and meet health requirements for flying. This will include annual weight, blood pressure, hearing, and vision screening. Transport team must meet height requirements of not greater than 6ft 2in., not less than 4 ft 10 in and maintain weight less than 215 pounds. Responsible for training and supporting home dialysis patients, including staffing a weekly home dialysis clinic. 1 Dialysis Technician – are an integral part of the outpatient dialysis unit operations. Dialysis techs set up machines, initiate and discontinue patient treatments and keep the outpatient dialysis unit stocked with supplies to assure the best care is given to our patients. 5 Nephrologists – Provided inpatient and outpatient coverage to patients that are on or needing dialysis services. Multidisciplinary Ancillary Staff : Social work, Nutrition, Psychology, Child Life, Transplant Coordinators and more provide ongoing support, education and other regulatory requirements to our patients and their families. Dialysis Unit: One Patient Training room Water treatment room is double the size as it used to be, with state of the art Reverse Osmosis System to provide Ultra Pure water for patient treatments 6 patient treatment stations (4 chairs in an open bay area and 2 private rooms) Outpatient Hemodialysis on Mondays, Wednesdays and Fridays, sometimes Saturdays Patient treatments average 3 to 4 hours each First shift starts at 0600 Treatment days end around 1700 On Tuesdays, nurses work in the Peritoneal Dialysis Clinic, where patients who perform their dialysis at home, come to see a multidisciplinary team. On Thursdays, nurses work to care plan HD patients and work on assigned projects and duties. All other days are covered by nursing to perform dialysis services as needed What you will do: Performs as a partner in the Dialysis Unit care management team. Provides coordination and management of patient care to pediatric patients and families within a continuum of care. Delegates patient care activities as appropriate within a care management assignment. Functions within professional and hospital standards. Performs essential nursing functions Assumes direct care responsibilities of a Dialysis Pediatric Nurse. Shares in the on-call rotation with other dialysis nursing staff. Nursing Process: applies principles of growth and development in obtaining assessment data; communicates in manner most appropriate for child and family; identifies and acts upon overt and covert cues of physical, psychosocial and cognitive parameters for an individual; makes independent nursing decisions and integrates medical plan of care. Understands and satisfies the needs of the neonatal, infant, toddler, pre-school, school-age, adolescent and/or adult patient in regard to their growth and development process. Care Management: collaborates with physicians and others to implement care management plans; delegates patient care activities within a care management assignment; assesses progress of patient toward established nursing outcomes; records variances; communicates with care team when outcomes are not met. Involved in regulatory preparedness for Centers for Medicaid and Medicare Services (CMS) regulations and continuous effort to maintain a state of readiness for ongoing compliance for unannounced surveys. Teaching: understands/applies basic learning principles to both children and adults; is aware of available resources; familiar with content in teaching plans; executes teaching plan; utilizes available resources, both written and consultant; evaluates effectiveness of own teaching; is accountable for the completion of teaching. Participates in Home Visits for patients that have or will train to perform home dialysis treatments to assess home environment and reassess need for re-training. Administers hemodialysis and peritoneal dialysis. Participates in Home Visits for any new peritoneal patients, or patients experiencing frequent infections or a recent move to location. Home visits are performed with 2 nurses, at a minimum, when going into the community. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Surgical-Medical Pediatric Critical Care- W4 Surgical-Medical Pediatric Critical Care Unit is a 44-bed multispecialty intensive care unit. Nurses here care for the immediate postoperative needs of patients requiring intensive airway management and physiologic monitoring such as those recovering from neuro-surgical procedures, trauma, solid organ transplant, and other surgical procedures. In addition to the surgical population, a variety of medical patients are cared for including chronic needs patients and patients with tracheotomies requiring extensive teaching to be able to be discharged home. Due to the nature of the patients cared for on this unit, there is a high volume of admission, discharge and transfer activity. Nurses on West 4 are extremely skilled in a wide-variety of competencies from neurological dysfunction, solid organ transplantation, dialysis , respiratory ailments, and are able to care for patients with surgical/trauma needs, chronic conditions and much more. Patient population on this unit may also include respiratory illness, sepsis, or be a bone marrow transplant or oncology patient requiring an intensive level of medical and nursing management. Under the direction and supervision of a Registered Nurse, performs nursing tasks for which he/she has been trained and has demonstrated competency through a skills checklist. Senior level status in any nursing program required. Ability to communicate and understand detailed instructions relating to basic patient care and transport. Regularly attends NNP meetings/conferences. Collaborates with the entire team to optimize patient care delivery and patient safety during system changes. Advocacy/Moral Agency Identifies ethical conflicts or issues that may surface in clinical setting, and prepares patient and family as well as clinical team to address them. Establishes an environment that promotes ethical decision-making and patient advocacy. Provides patients and families with appropriate information to help them participate and/or make informed decisions about health care and treatments, including health promotion and disease prevention.
