We provide specialized care to patients recovering from stroke, orthopedic procedures, neurological conditions and other life altering trauma. You would be joining a collaborative unit that includes a team of Physical Therapists, Occupational Therapists, Speech Therapists and Physiatrists. This role focuses on providing high quality, patient-centered care. We are dedicated to helping patients regain independence and improve quality of life. KEY ACCOUNTABILITIES: Utilizes the nursing process, evidence-based practice, and specific competencies to assess the physical condition and nursing needs of patients, and develops a plan of care in a collaborative practice with the patient and interprofessional team. Plans for the care needs of the patient in collaboration with the interprofessional team to provide the highest quality of care and clinical outcomes. Performs and document tests or procedures including, but not limited to complex dressing changes, intravenous therapy interventions, invasive specimen collection, and others within scope of licensure. Assists in discharging the patient at the completion of the office visit reinforcing the after-visit summary. Aids in the completion of medication, laboratory, and radiology orders as needed. Demonstrates professional and therapeutic relationships with patients, families, team members, and providers. Participates in patient panel management. Works with care team members to monitor patients who would benefit from recommended health screening, preventative services, and established chronic disease tracking interventions. Inform patient of results. Other duties as assigned. QUALIFICATIONS: Current Wisconsin LPN Licensure American Heart Association Healthcare Provider Basic Life Support (BLS) PHYSICAL DEMANDS: Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of Fifty (50) pounds without assistance Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties WORK ENVIRONMENT: Frequent exposure to sharp objects and instruments. Occasional exposure to moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electrical shock Occasional high noise level in work environment Standing and/or walking for extended periods of time Transporting patients and/or equipment from one location to another; little likelihood for injury if proper body mechanics and procedures are followed Possible exposure to communicable diseases, hazardous materials, and pharmacological agents Occasional contact with aggressive and or combative patients.
