Job Summary: SUNY Upstate is seeking a highly motivated Retention and Adherence LPN Specialist to provide services to patients seen in the Inclusive Health Services and the Pediatric Infectious Disease Clinic's grant funded retention and adherence program. Position provides linkage to and retention in medical care as well as Intensive Case Management to a specific cohort of patients, those who are new to antiretroviral therapy and/or who have not achieved viral suppression. Primary Responsibilities: Assist patients in identifying the barriers preventing them from engaging in medical care and from attaining a suppressed HIV viral load. Work with the patients to create a plan to address these barriers. Encourage patients in reaching their goals and follow up with them as needed. Services will improve the health outcomes for the patients and decreasing the community viral load through prevention for positives.
