Description Position Summary: Primary responsibility is to deliver and coordinate patient care on the assigned shift and provide leadership to the staff with direct oversight of LPNs and LNAs. Qualifications: Education/ Experience: Graduate of an accredited school of nursing. Current Vermont RN license in good standing. AHA BLS certification required. Other Requirements: Computer skills and familiarity with using an electronic medical record essential. Excellent customer services skills. Strong team player. Requirements Essential Functions Of The Position: Provides care, treatments, and services within the RN scope of practice. Implementation of provider’s orders to include medication administration and treatments. Consults and coordinates with health care team members to assess, plan, implement and evaluate patient care plans. Prepares and administers prescribed medications with appropriate documentation. Reports adverse reactions to medications or treatments in accordance with the policy. Identifies psychosocial needs of patients and family and incorporates them into the patient’s care plan. Educates patients on medical procedures and patient’s personalized plan of care. Provides bedside care for a variety of medical patients. Initiates patient education plan. Teaches patients how to manage their care. Complies with Pyxis Medication Dispensing policies and protocols. Maintains POC competency and documents per hospital standards and policies. Assist Providers with procedures as requested and within scope of practice, demonstrating good technical and manual skills. Consults other departments, as appropriate, to provide for an interdisciplinary approach to the patient’s needs. Aware of available support services. Is flexible, organized, and able to function in stressful situations. Performs all aspects of patient care in an environment that optimizes patient safety. Manages and operates all equipment safely and correctly. Records patients’ medical information, vital signs and care information concisely, accurately and completely, in a timely manner.

As a Hospitalist at Grace Cottage, you will be responsible for providing comprehensive medical care to patients admitted to our facility. Key Responsibilities: Admit, evaluate, diagnose, and treat patients in the hospital. Manage and oversee the care of hospitalized patients from admission to discharge. Conduct daily rounds and participate in multi-disciplinary team meetings. Description Job Summary: Primary responsibility is to provide strong, prioritized patient care in a variety of urgent or emergent situations while maintaining a global perspective of the department. Qualifications: Current and maintained Paramedic Certification or Critical Care Paramedic level or graduate of an accredited school of nursing, BSN preferred. Current Vermont RN license in good standing. Two-three years’ experience in Emergency Nursing. Current BLS, ACLS, PALS, required. TNCC, ENPC, and CEN preferred. Other Requirements: Computer skills and familiarity with using an electronic medical record essential. Excellent customer services skills. Strong team player and ability to function autonomously. Must be willing to work nights. Requirements Essential Functions Of The Position: Performs triage using the Emergency Severity Index, while considering both physical and psychosocial elements to determine the degree of urgency for treatment. Maintains a strong understanding of the Incident Command Structure and can operate fluidly within it, maintaining patient and facility safety during an internal or external crisis. Collects current symptoms, as well as a detailed patient history and then consults and coordinates with health care team members to assess, plan, implement and evaluate patient plans of care and treatment. Manages basic life support needs, based upon GCFHH nursing standards and protocol. Monitors and adjusts specialized equipment used on patients, and interprets and records electronic displays including but not limited to; Cardiac monitoring, EKG monitoring, respiratory ventilators, non-invasive positive pressure machines and high flow nasal cannula. Initiates corrective action whenever information from monitoring equipment shows adverse symptomatology within the GCFHH nursing standing orders in addition to notifying the Provider. Records patients’ vital signs and all care information concisely, accurately and completely, in a timely manner, in the appropriate format and on the appropriate forms. Works directly under Providers, assisting them during exams, diagnostic testing and treatments Prepares and administers medications and records medication administration appropriately, including scanning patient identification and medications. Reports adverse reactions to medications or treatments in accordance with the policy regarding the administration of medications by a licensed registered nurse. Maintains proper supplies and appropriate medical equipment to care for patients. Initiates patient education plan, as prescribed by physician. Teaches patients and significant others how to manage their illness/injury, by explaining: post-treatment home care needs, diet/nutrition/exercise programs, self-administration of medication and rehabilitation, as well as provides referrals to other healthcare professionals for follow-up treatment.
