Position Summary The Licensed Practical Nurse (LPN) (Float) provides direct patient care in accordance with applicable laws and organizational policies. The LPN supports the delivery of compassionate, high-quality care in hospice and palliative care settings, performing assigned clinical tasks and collaborating with interdisciplinary teams to ensure optimal outcomes for patients and caregivers. Essential Functions Assist in implementing and modifying patient care plans in collaboration with the RN and interdisciplinary team. Deliver patient-centered care, including administering medications and performing procedures within the LPN scope of practice. Monitor and report changes in patient condition to the RN and physician; document accordingly. Educate patients and caregivers on appropriate self-care practices. Participate in discharge planning and team meetings. Maintain patient confidentiality and ensure compliance with HIPAA regulations. Manage and order medical supplies as needed. Facilitate communication between care team members and physicians. Support organizational initiatives such as disaster preparedness and performance improvement activities. Administer IV therapy as permitted by licensure and training. Follow general safety and infection control procedures. Participate in total organization program activities including: • Demonstrating professional relationships with internal and external customers • Promoting the organization’s image • Using chain of command for problem solving • Demonstrating appropriate dress and grooming • Displaying courtesy, tact, patience, and respect • Maintaining good attendance and notifying of absences per policy • Supporting all departments cooperatively • Participating in QAPI activities • Following Clinical Care standards Knowledge and Critical Skills Knowledge of hospice nursing practices and principles Strong patient observation, documentation, and communication skills Ability to adapt to changing environments and schedules Excellent interpersonal and organizational abilities Capable of working with diverse populations across age, race, and health conditions Demonstrates sound judgment and maintains confidentiality Experience Minimum of two (2) years of patient care experience required Acute care and hospice nursing experience preferred License / Certification Current Florida LPN license IV Certification (Florida preferred) Current CPR certification Education Graduate from an accredited Florida Board of Nursing program for Licensed Practical Nurses Other Requirements Must be willing to work at various hospice centers or teams as needed Participation in organizational quality and safety programs is expected The knowledge, skills and abilities listed above are typically acquired through the levels of education and experience listed. The Licensed Practical Nurse (LPN) provides direct patient care in accordance with applicable laws and organizational policies. The LPN supports the delivery of compassionate, high-quality care in hospice and palliative care settings, performing assigned clinical tasks and collaborating with interdisciplinary teams to ensure optimal outcomes for patients and caregivers. Position Details Days/Hours : Nights; 3x12 shifts, 8pm-8am weekdays with some weekends, usually one weekend a month Work Coverage Area: Alachua County area; Gainesville and Lake City; home care with some local Assisted Living Facilities Certification Required: ** IV Certification ** Compensation : $27.00 - $32.00/hr, plus extra paid driving mileage; full benefits package plus 23 days annual PTO Sign On Bonus Eligible : Yes $2,000 Primary Job Responsibilities Completes assessment of patients regularly and on-going as patients’ conditions warrants. Provides patient care in accordance with physician orders and patient/family goals, directed by the Care Plan; provides post-mortem care per policy. Demonstrates awareness and maintenance of Universal Precautions. Provides emotional support to patient/caregiver. Teaches patient/caregiver as appropriate to situation, plan of care. Demonstrates knowledge and appropriate use of medications, including medication administration. Demonstrates competency in skills identified, maintaining completion of annual competency of these skills per agency standard. Follows company policies and guidelines related to specific job functions. Other duties as assigned. Coordinates implementation of Plan of Care so that: Communicates timely, thoroughly, professionally with Medical Director, Home/LTC Team Manager/designee, Inpatient unit team members (as appropriate), Home and LTC teams, patient/family, other Community Hospice departments. Evaluates services provided to patients, apprising Inpatient Unit Manager /designee if compliments/concerns or opportunities for improvement are expressed. Facilitates transfers into the Inpatient Facility (if appropriate) further ensuring continuity of care of Hospice services. Completes documentation so that: Charts patient condition/changes every 1 hour during shift. (For CIC cases only). Documentation completed is reflective of patient care given, services provided, relevant to Continuous Care criteria and death/discharge process. Any concerns noted, patient status; is legible, neat; is accurate, thorough, completed in a timely manner. Documents relevant to the Medication Administration, Narcotic Administration, and accountability and destruction of medication following patient death. Care Plans are kept current, applicable to patient and patient needs. Incident Reports are completed accurately and submitted immediately to the Nursing Support Service Manager/designee. Licenses & Certifications IV Certification required Valid LPN License State of Florida driver’s license with driving record acceptable to Community Hospice insurance carrier. Current BLS Education & Experiences Any combination of education and/or experience that would provide the required skill and knowledge for successful performance would be acceptable. Typical qualifications would be equivalent to: Graduate from an accredited Practical Nursing Program Minimum of one (1) year experience in acute care setting, long term care setting or home health setting as an LPN. Reliable transportation that allows employees to shift work locations when needed. Furnish one or more active telephone numbers that enables management to contact employee should an unexpected change of work location be required.

Jacksonville, FL 32257Under the supervision of the Triage manager and the direction of the Triage supervisors or Team managers, provides patient care in a variety of settings for the purpose of assessment, symptom management, treatment, teaching, emotional support, coordination of care, deaths, coverage of routine case management visits, etc. Work hours include every Saturday and Sunday from 8am-8pm and one weekday (either Monday or Friday) from 830a-5p, based on organizational needs. Primary Responsibilities: Weekend coverage: Performs skilled nursing activities such as: Completes assessment of patient for effective symptom management. Incorporates nursing skills, communication skills, and emotional intelligence skills to effectively assess problems/needs. Perform treatments and interventions in accordance with the plan of care and physician’s orders. Provide information and education regarding disease progression and anticipated needs at end of life. Provide instruction to the patient/care giver in accordance with the plan of care or changes in plan of care. Collaborate with patient, family/caregiver, and practitioner regarding goals of care and changes in treatment plan. Follows precautions based on plan of care. Provides appropriate emotional support during all conversations with patients/caregivers. Conducts follow up and collaboration calls as needs arise or as requested by the team. Communicates and coordinates as follows: During shift answers phone at time of call or returns call to Triage within 5 minutes (unless actively engaged in patient care). Provides real-time updates to supervisor regarding visit arrival and departure times per protocol. Provide change of status updates to supervisor (such as GIP/ER transfer or death) Call physician or APRN as necessary for change in plan of care. Facilitates change to continuous care and GIP transfer, per policy. Collaborate with supervisor or IDG team to determine appropriate discipline to respond to call and/or visit need. Arranges for medications and supplies based on urgency, per policy. Communicates with staff, physicians, community resources, and vendors providing accurate information regarding patient’s needs/problems. Documents all phone calls or in-person encounters with patients/caregivers in the EMR in a timely manner per CHPC protocol. Leaves accurate and detailed report, per policy, to IDG team members, after all visits including death visits. Weekday coverage: Act as a member of an assigned care team, coordinating resources and performing a wide range of skilled nursing activities for an assigned caseload of adult patients or to assist the wider team, as directed by the home or LTC team manager Conduct assessment of patient/caregiver needs; symptom management; teaching of appropriate care related to disease process; and participating as a member of the interdisciplinary group (IDG) in providing end-of-life care/support for patients and caregivers alike. Performing skilled nursing activities as indicated above Communicate and collaborate with the IDG team as directed by the team manager Provide end of shift report as directed by team manager Minimum Qualifications: Possession of a degree from a diploma nursing program or a Bachelor’s degree in Nursing. Minimum of two (2) years experience in acute care setting, oncology, or home health care setting. Skilled at t eaching, communication and listening skills, assessment skills, central/peripheral IV skills, working within the framework of a multidisciplinary team. Required Licenses/Certifications: Current license as a Registered Nurse in the State of Florida. Current CPR certification Current State of Florida driver’s license with driving record acceptable to Community Hospice of Northeast Florida insurance carrier and reliable transportation. Physical Requirements/Work Environment: Home care position for 8 to 12 hour shifts. Use of own vehicle required.
