Responsibilities Supervise and coordinate the care of patients on weekends, ensuring that all medical and non-medical needs are met in a timely and efficient manner. Lead and mentor a team of RNs to deliver high-quality patient care, promote collaboration, and foster a positive work environment. Develop and implement patient care plans , incorporating input from multidisciplinary teams, and monitor patient progress to achieve optimal outcomes. Collaborate with other departments to ensure seamless communication and coordination of care, including admissions, discharges, and transfers. Perform administrative tasks , such as charting, reporting, and quality improvement initiatives, to maintain accurate records and support continuous quality improvement. Ensure compliance with regulations , laws, and company policies, maintaining a safe and respectful environment for patients, families, and staff. Certified Nursing Assistant (CNA) – Skilled Nursing / Long-Term Care Position Summary: We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our skilled nursing and long-term care team. The CNA provides hands-on resident care, supports daily living needs, and helps create a safe, respectful, and engaging environment focused on resident comfort and quality of life. Responsibilities: Assist residents with activities of daily living, including bathing, dressing, grooming, toileting, feeding, and mobility needs Provide compassionate, respectful, and person-centered care while promoting resident dignity and independence Support residents with transfers, repositioning, and comfort measures as needed Collaborate with nursing staff and the healthcare team to support individualized resident care plans Observe and report changes in residents’ physical, mental, or emotional condition to nursing staff promptly Assist with maintaining accurate documentation of resident care and services provided Encourage resident participation in activities and programs that promote engagement, socialization, and overall wellness Promote a safe, clean, and comfortable environment for residents, staff, and visitors Follow infection prevention practices and facility standards of care Requirements: Current Certified Nursing Assistant (CNA) certification in the State of Oklahoma Previous experience in skilled nursing, long-term care, or geriatric care preferred Strong communication and teamwork skills Ability to provide compassionate care and work effectively with residents, families, and staff Ability to lift, transfer, and reposition residents as needed Join our team and help make a difference in the lives of our residents every day!
