Assembles and uses such equipment as catheters, tracheotomy tubes, and oxygen suppliers. Collects samples, such as urine, blood, and sputum, from patients for testing and performs routine laboratory tests on samples. Sterilizes equipment and supplies. Prepares or examines food trays for prescribed diet and feeds patients. Records food and fluid intake and output. Bathes, dresses, and assists patients in walking and turning. Cleans rooms, makes beds, and answers patients' calls. Washes and dresses bodies of deceased persons. Assists in delivery, care, and feeding of infants. Inventories and requisitions supplies. Performs tracheotomy care, colostomy care, wound care and dressing changes. Demonstrates through clinical practice the use of universal precautions, understands the OSHA Bloodborne Pathogens Standards. Purpose of Position: The Primary Public Health Nurse is a registered professional nurse who organizes clinical aspects of town and corporate contracts. The Public Health Nurse along with the Public Health Manager promotes prevention services for persons of all ages throughout our service area, including communicable disease, emergency preparedness for a pandemic occurrence, immunization clinics, community health clinics and health education and screening programs. Works collaboratively and professionally with other members of the health care team to ensure quality of services and patient safety. Maintains knowledge of current agency services and criteria for admission. Maintains current knowledge of agency policies, procedures, objectives and current documentation requirements to meet regulatory standards. Communicates effectively and professionally on e-discharge systems. Efficiently and accurately enters data into the EMR. Completes Insurance Verification process for referrals entered, ascertains and documents coverage criteria. Provides information and education regarding Agency services and programs to the public. Triages telephonic requests for services as needed. Participates in Agency, clinical, QI, liaison, clinical reimbursement and case conerences activities as required. Other duties as assigned. Consistently provides service excellence to all patients, family members, visitors, volunteers and co-workers. Challenges current working practices; identifies process improvement opportunities and presents recommendations and solutions to management. Engages and commits to the organization’s culture of continuous improvement by actively participating, supporting, and promoting CCHC Pillars of Excellence. Qualifications RN currently licensed in Massachusetts (job code RNCT/RNCTPD) or LPN currently licensed in Massachusetts (job code 10611). 2 years’ experience in healthcare, homecare experience preferred. Experience with EMR and E-Discharge platforms preferred. Ability to multi-task and manage multiple aspects of the referral process.
