Transcribes physician’s orders as per p/p; ensures all orders are written correctly and have appropriate dx. Monitors hydration of residents, including accurate I & O. Notifies dietician and MD if not meeting fluid needs or exceeding needs and documents same in residents’ chart. Makes rounds during mealtime to see that each resident has received help as needed, assists in hand feeding, assures that sufficient fluids, special feedings and nourishments are administered as needed. Completes assignments timely, i.e.: quarterly evaluations Coordinates activities with ancillary departments to ensure that treatments plans are followed and/or revised as needed. (i.e.: therapy) Utilizes equipment properly and ensures direct care staff does same; communicates to RN supervisor if education is needed. Monitors weight of all residents, utilizing protocol if there is a significant weight loss. Ensure plan of care is being followed. Cooperates with all staff; is respectful to coworkers.
