RN/LPN MDS Coordinator We are looking to hire an RN/LPN MDS Coordinator! Job Duties: · Oversee the day-to-day patient care, supervising, directing and developing nurse staff. · Coordinates the care plan according to regulatory requirements. · Ensure that resources are made available to patients and that patient care is delivered effectively and to a satisfactory standard. · Create the schedule for all Medicare and Medicaid. · Start Medicare coverage for newly qualified patients or send out denial letters and remain updated on changes in Medicare coverage and help determine documents needed for reimbursement. Required Skills/Abilities: · Good decision making and judgment capabilities. · Ability to identify and solve complex problems. · Passionate about making a difference, connecting with people. Licensed Practical Nurse (LPN) Full Time (7a-7:30p) We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our growing team! The Licensed Practical Nurse (LPN) is responsible for providing direct patient care under the supervision of a registered nurse or physician. The LPN will be responsible for administering medications, taking vital signs, performing wound care and assisting patients with daily living activities. The LPN will also be responsible for documenting patient care in the electronic health record and liaising with other healthcare professionals as needed.
