P rovides one hour of pre-chemotherapy/immunotherapy education session to Oncology patients when Nurse Practitioner (NP) unavailable. C ollaborates with interdisciplinary professional staff, patient, family and caregivers to ensure appropriate clinical follow-up plan and continuity of care. C linical resource on chemotherapy/immunotherapy administration for inpatient nurses. Provides 4-hour clinical introduction to chemotherapy/Immunotherapy administration to inpatient nurses. M aintains records, inventory and physical security of drugs, medical supplies. Orders and restocks supplies as needed. A dheres to infection control practices to maintain a safe Infusion Center environment. Manages the interface between the patient and medical technology to ensure a safe, respectful and humane environment. Summary Under limited supervision of the Director of Cardiac Device Clinic, the Cardiac Device nurse is independently accountable for the effective management and coordination of care of patient with Cardiac Implantable Electronic Device (CIED), pacemakers, ICD and Implantable Loop Recorders. The RN will partner with Cardiologists, Cardiology APPs and other clinical staff throughout ANTHC to coordinate the care of cardiovascular patients with CIEDs. The Cardiac Device RN will be responsible for providing comprehensive patient care by completing thorough, competent interrogations and reprogramming of implanted cardiac device in the inpatient and outpatient settings. The Cardiac Device RN will assist in coordination of care and device optimization throughout imaging studies. The Cardiac Device RN is responsible for triaging remote transmissions and programming recommendations for patients undergoing therapies that may impact the device. Responsibilities Conduct interviews with patient, family and other health care agencies to obtain past medical records and social and medical history to assess their understanding of the plan of care regarding cardiac monitoring device. Performs independently perform CIED interrogation and device/patient monitoring in the inpatient and outpatient setting, including MRI, ER per cardiology provider’s orders Provides comprehensive patient care by accurately identifying malfunctions, alerts, and sub-optimal device operations along with clinical scenario and follow up appropriately with the patient and provider teams. Manage patient enrolled in remote monitoring services. Coordinate plan of care by triaging alerts and communicating with provider’s aligned nursing teams Proactively review patient schedules to ensure efficiency in clinical workflows and identify and resolve potential conflicts. Ensure appropriate resources are available to support patient care and procedures in both inpatient and outpatient settings. Educate patient and family on signs and symptoms that require a follow up telephone call or clinic visit.
