This position is responsible for providing administrative and clinical support to ensure efficient delivery of primary health care and creating a positive patient experience. Full-Time/Benefits Eligible Monday - Thursday - 6:45am-3:30pm, Friday - 6:30am-12:00pm Columbus, OH ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: • Work closely with the clinical care team to deliver the best care for our patients. • Foster a climate allowing for direct communication between the care team, patient/caregiver and appropriate care providers to optimize outcomes. • Collaborate with other care team members to address gaps in care (chart prep). Record vital signs and other health-related issues on patients’ charts. • Assist physician or RN in patient care assessment of health status in order to provide patient care. Assist in performing routine medical procedures as requested. • Review physician orders and lab requests and follows up as needed. Call in prescriptions/order refills as approved and directed. • Communicate test results to patients as directed (includes reading TB results) and record notes in Electronic Health Records (EHR). • Assist in patient education with related educational tools, periodicals, flyers, and handouts. • Assist with Department of Transportation, FMLA, and Medical Disability application forms. • Respond to patient phone calls, as appropriate. • Maintain a clean and well-organized work area. • Maintain patient confidentiality in alignment with HIPAA regulation. • Attend meetings and participate in classroom activities as needed. • All other duties as assigned. The Palliative Care Intake RN supports the Comprehensive Care team by triaging incoming referrals and assigning to appropriate clinical disciplines and providers. Central Ohio Primary Care is seeking a full-time Advanced Practice Provider (NP) for True Vine Family Medicine at 201 N. Plaza Blvd., Chillicothe, OH 45601 The APP, in collaboration with the Physician, coordinates the health care of primary care patients at this location. This position monitors the patient’s status and collects data to evaluate and improve care by providing a comprehensive assessment, implementing the appropriate treatment, and evaluating the patient’s progress. Key Responsibilities: Conduct thorough assessments, including medical history reviews and physical examinations, to diagnose and treat acute and chronic health conditions in patients of all ages. Provide a full spectrum of primary care services, including well visits, health promotion, disease prevention, routine screenings, vaccinations, and management of common illnesses and injuries. Develop individualized treatment plans for patients with chronic diseases such as diabetes, hypertension, asthma, and COPD. Monitor disease progression, adjust treatment regimens, medication management and educate patients on self-management strategies. Address questions, concerns, and health-related inquiries to promote informed decision-making and adherence to care plans.
