Job Description ATLANTICARE _____________________________________________ROLE DESCRIPTION The Clinic Licensed Practical Nurse (LPN) is an integral part of the multidisciplinary team at the hospital-based clinic. Collaboration with Physicians, Advanced Practice Providers, Nursing, and MA’s is imperative to the success of the clinic. The Clinic LPN utilizes the care process consistent with their license and within their scope of practice. The LPN provides direct nursing care to patients under the supervision of the Providers (Surgeons and Advanced Practice Providers) in a trauma, surgical and orthopedic clinic setting. The LPN is responsible for supporting patient care related to acute injuries, pre-operative and post-operative follow-ups, and chronic orthopedic conditions, ensuring a high standard of clinical and patient-centered care. The LPN is responsible for performing the following tasks, but not limited to: Prepare exam rooms, stock supplies, and maintain cleanliness and infection control standards. Complete intake forms, vital signs, wound care, dressing changes, suture and staple removal, casting and splinting assistance, and orthopedic device education. Assist with trauma and orthopedic procedures, pre- and post-operative care, and patient positioning. Assist surgeons and providers with in-clinic procedures, including local anesthetic administration, suture removal, drain management, and sterile field preparation. Provide patient education on medications, wound care, mobility aids (e.g., crutches), and post-discharge instructions. Support physicians and advanced practice providers during examinations and treatments. Coordinate pre-op and post-op testing (labs, imaging) and ensure all pre-surgical and post-surgical requirements are completed. Maintain accurate documentation in electronic health records (EHR) and ensure timely charting of patient care. Collaborate with multidisciplinary team members to coordinate care and follow-up appointments. Respond to patient questions and concerns with professionalism, compassion, and clinical expertise. Administratively, the Clinic LPN provides clerical support to the department director with various functional responsibilities and assists with departmental operations as required. The LPN collaborates with the patient access team to ensure timely and accurate financial clearance of all clinic accounts. The LPN is responsible for scheduling procedures and clinic visits. The LPN works in collaboration with the Medical Assistant to ensure seamless transitions in patient care. JOB DESCRIPTION " Position Summary The Transition of Care Nurse provides consultative and coordinating functions in the transitioning of patients from the acute care facility to home or alternative care sites. This position works in conjunction with the Nursing and Medical/Case Management departments of the hospital and at the direction of the management of AtlantiCare LIFE Connection. The Transition of Care Nurse also is responsible for the coordination of care of all AtlantiCare LIFE Connection participants currently residing in an acute care setting. Position Summary JOB DESCRIPTION Responsibilities The Registered Nurse Health Services is a member of the multidisciplinary team working in collaboration with the physician. The RN utilizes the care process consistent with their license or certification including initiation of interventions within individual scope of practice. The RN is also responsible for overseeing any necessary coordination of the multidisciplinary plan of care and provides for its consistency with the medical treatment plan. The patient care process includes: Assessment- an orderly process consisting of the identification, gathering and organization of subjective and objective data pertaining to the patient/family/significant other. Planning- an organized step in the process that consists of the identification of the actual or potential problems and desired outcomes (goals) that result in a documented plan of care. The plan of care provides direction for the patient/family/significant other. Intervention- the specific interventions or orders that are implemented consistent with the plan of care and medical treatment plan. Evaluation- a step in the process that involves determining the effectiveness of the interventions in achieving the plan of care and determining the conclusions concerning the need for reassessment and revisions to the problems, goals, plan of care, and interventions. This step provides for documentation of required actions. Based upon findings identified during assessment and reassessment: Patients have problems identified and prioritized relative to biophysical, psychological, environmental, self-care, educational and social status.
