Job Duties Systematically and continuously collects, records, and appropriately communicates data regarding the patient's health status, integrating knowledge of age specific parameters Performs initial physiological needs assessment within established time frames Accurately completes the admission assessment form within 24 hours of patient's admission. Derives appropriate nursing diagnosis from identified problems of the individual based on data collected Reassess and documents the patient's total needs per established unit policy Routinely reviews assessment data, identifies educational needs, and initiates discharge planning Clinical Performance Directly supervises and evaluates the job performance of assigned team members, exercising skill in planning and delegating. Performs appropriate treatments as ordered by the physician in an accurate and timely manner; following established policy and procedures. Follows infection control procedures and universal precaution guidelines in all aspects of patient care. Reports immediately any finding of vital signs, behaviors, subjective complaints or objective observations that may indicate patient health deterioration. Initiates emergency resuscitative measures according to ACLS guidelines and/or established protocols. Completes reports, charging, clinical forms, and quality reviews accurately, legibly and within established timelines. Reviews articles, manuals, minutes, and policies needed or requested to be knowledgeable and remain current. Exercises decisions making and prioritizing skills as a response to constant changes in work load and care demands. Administers medications as prescribed, monitors for adverse effects; and documents in compliance with P/P regulations. Documentation and Evaluation Evaluates and documents patient responses to all interventions and procedures carried out both on a physical and psychological basis Consults, re-evaluates, and revises multi disciplinary plan of care based on patient's progress and behaviors Documents medication administration per unit policy Assesses and documents the effectiveness of all PRN medications within l hour of administration Documents changes from baseline assessment Documents verbal orders on the physician order sheet and ensures co-signature in a timely manner per policy Accurately and legibly documents using hospital approved abbreviations, appropriate signatures, titles, error corrections and black ink Appropriately documents and completes occurrence form in a timely manner Assures completeness of nursing documentation on the medical record at the time of discharge Patient Education Utilizes assessment data to develop an educational plan in conjunction with the patient, significant others, and/or other disciplines to effectively meet the identified needs Actively participates in patient education by appropriately utilizing available resources and adjusting it to meet the patient's individual needs and level of understanding Evaluates and documents patient and/or significant others response to education given Documents discharge teaching information given Considers patient's cultural and religious practices, emotional barriers, physical/cognitive limitations, and language when planning educational interventions and documents on educational record consistently Planning of Patient Care Initiates plan of care reflecting the assessment of the immediate physiological needs of the patient within established time frames Develops plan of nursing care in conjunction with the individual and/or significant others and includes expected outcomes appropriate for the nursing diagnosis and unique patient/family needs Prioritizes and prescribes appropriate nursing measures to achieve the expected outcomes Evaluates, modifies, and updates the nursing care plan based on observable patient responses and attainment of patient outcomes within established time frames Organizes, plans and documents discharge needs in the care plan Education Associate's degree in nursing from an accredited institution required, bachelor's preferred Additional education requirements Graduate of an approved Nursing Program (BSN preferred) Certification & Licensures BLS Required ACLS Required Additional Certification & Licensures details Active Tennessee RN license or compact state. BLS required by hire date ACLS, CPI and Chemotherapy and Biologic Administration Competency required within 90 days of hire date. About Our Kids Center: The Our Kids Center specializes in providing forensic medical evaluations for children when there are concerns of child sexual abuse. Serving approximately 750 children in Middle Tennessee each year, Our Kids is a recognized leader in forensic medical evaluations and research in this field. The successful candidate will be a PRN employee at Nashville General Hospital and assigned to the Our Kids Center. LDRP RN Roles and Responsibilities: Assessment: Conducts comprehensive assessments of the laboring mother, including fetal monitoring, pain management, and maternal vital signs. Assesses newborns immediately after birth, ensuring proper identification, Apgar scoring, and initiation of necessary care. Care Planning: Develops individualized care plans based on patient assessments and collaborates with the healthcare team to ensure continuity of care. Labor Support: Provides continuous labor support, including emotional support, education, and guidance on various comfort measures and pain management techniques. Delivery Assistance: Assists with vaginal deliveries and cesarean sections, including acting as a second assist to the physician. Prepares and maintains sterile fields, administers medications as ordered, and provides immediate postpartum care to both mother and newborn. Recovery and Postpartum Care: Monitors mothers and newborns during the recovery period. Assists with breastfeeding or formula feeding. Provides education on newborn care. Addresses any concerns or complications Newborn Care Provides expert care for newborns, including routine screenings, vaccinations, and monitoring for jaundice and other common neonatal conditions. Facilitates early skin-to-skin contact to promote bonding and breastfeeding success. Supports parents in understanding and responding to their newborn's cues and needs. Level II NICU Care: Provides specialized care for newborns requiring Level II NICU interventions, including respiratory support, intravenous therapy, and feeding assistance. Monitors vital signs, administers medications, and performs procedures as ordered by the neonatologist or neonatal nurse practitioner. Educates parents on the specialized care their newborn is receiving and fosters a supportive environment for the family. Medication Administration: Administers medications safely and accurately, adhering to hospital protocols and physician orders. Documentation: Maintains accurate and comprehensive documentation of all patient care activities, including assessments, interventions, and patient responses. Patient Education: Educates patients and families on labor progress, pain management options, newborn care, postpartum self-care, and available resources. Collaboration: Collaborates effectively with physicians, midwives, lactation consultants, and other healthcare professionals to provide seamless and holistic care. Emergency Response: Responds promptly and effectively to emergencies, including fetal distress, postpartum hemorrhage, and newborn resuscitation. Required Skills & Qualifications: Graduate of an accredited Associate Degree Diploma or Bachelor's Degree program in Nursing Current and unrestricted Registered Nurse (RN) license in Tennessee A minimum of 2 years of recent experience in L&D, PP and NICU Level II/Nursery area is preferred but cross-training will be provided for those proficient in one or two of those areas. Recent experience in an acute care facility is highly desirable Experience with quality/performance improvement programs utilizing clinical pathways/guidelines is a significant asset Knowledge of the Joint Commission, CMS and other regulatory standards is preferred Ideal candidates must be able to link quality of care/services to patient satisfaction, financial, and other statistical data. Must possess excellent computer and interpersonal skills and demonstrate the ability to educate staff and physicians effectively. Surgical OB Assist is required in position- training available within 30 days of hire BLS and NRP certifications required.
