Participates in daily patient care (unit) huddles, in some case ensuring that huddles are taking place along with weekly clinic staff informational huddles. Maintains all licensure and/or certification requirements for continuing education and best practices. Identifies, in collaboration with Charge Nurse, supports and participates in quality strategies in collaboration from Charge Nurse to evaluate compliance with evidence-based guidelines, standards, and to identify opportunities to improve patient outcomes. Ensure all tasks provided and associated with patient care, patient administrative processes and related duties comply with all regulatory and accreditation standards including The Joint Commission and CommUnityCare Standard Operating Procedures and CommUnityCare Policies and Procedures. Act as the site lead in the absence of the Nurse Manager Operations and/or Practice Leader. Medical Assistant

Overview: In collaboration with patients, families (as defined by the patient) and staff across all disciplines and departments is responsible for performing selected clinical duties including administrative/clerical duties, basic patient care and/or performing administrative tasks associated with patient intake. Responsibilities: Essential Duties: • Accurately interview patient and record patient history and measure and record vital signs such as height, weight, BMI, head circumference, current medications, pulse oximetry level and patient comfort level. • Verify patient information such as, but not limited to demographics, insurance, vitals and medications; complete intake process including verifying and entering patient data into EHR/EPM. • Perform duties such as greeting and assisting patients in a courteous and professional manner, answer telephone and provide information as necessary, verify patient information, etc. • Collect, perform, document, and report results of CLIA waived lab tests including, but not limited to: urine dip, pregnancy tests, finger stick blood glucose levels, fecal occult blood tests, rapid influenza tests, and rapid strep tests. • Collect, prepare and document specimen collection including, but not limited to: blood, urine, stool, and sputum. • Perform clinical duties as assigned including, but not limited to: preparing patients for examinations and /or treatments, assisting provider with exams or procedures, setting up exam or procedure rooms, and preparing instruments for sterilization. • Complete pre visit planning to support the clinical visit. • Administer medications per CUC formulary. • Perform medical acts as prescribed by Standing Delegation Orders. • Dismiss patient at end of clinic visit including completing the orders documented in the orders module, tasks, and patient plan. • Participate in orientation of new employees and attend mandatory meetings. • Work as a team member, interacting with staff, patients and providers to communicate and coordinate patient care and work flow effectively. • Coordinate and manage patient care including communicating test results to patient, assist with pre-certifications, referrals, authorizations, prescriptions and scheduling of patients for clinic, hospital and other ancillary medical treatments and tests. Follow up on diagnostic test to ensure that test have been completed and documentation is completed in EHR. • Perform surveillance programs to include but not limited to complete medical records including labs, authorizations, X-Rays, hospital records and any previous care pertaining to each patient visit • Maintain a clean, functional environment including cleaning and disinfection of equipment, exam rooms and storage areas as per CUC guidelines • As assigned, may serve as a scribe accurately and thoroughly document medical visits and procedures as they are being performed by the provider, including but not limited to the following duties: o Patient medical history and physical exam, o Procedures and treatments performed by healthcare professionals, including nurses and physician assistants. o Patient education and explanations of risks and benefits. o Provider-dictated diagnoses, prescriptions and instructions for patient or family members for self-care and follow-up o Prepare referral letters as directed by the provider • Perform other duties as assigned. Qualifications: MINIMUM EDUCATION: • Graduation from high school or equivalent. • Medical Assistant Diploma and/or National Medical Assistant Certification verifying graduation from a Medical Assistant Program or equivalent military healthcare experience. Nurse Manager-Operations

Overview: The Nurse Manager is responsible for the development, performance, and management of clinical patient care for nursing, medical assistants. The Nurse Manager will work as part of triad leadership team in making decisions that best support clinical staff, extended care team members and providers to ensure patients obtain access to care that is timely, effective, efficient and safe. The Nurse Manager is responsible for the population health management of assigned clinic patient panel by performing regular analysis of population health data, improve clinical care workflows that lead to improvement of the quality of patient care and enhance cost effectiveness. The Nurse Manager will also provide oversight and direction related to staff education, onboarding and training of new hires with a focus on CommUnityCare's care model. Responsibilities: Essential Functions: Manages the clinical support team. Implements and provides oversight of clinical care delivered by nursing, medical assistants, and ancillary care team. Establishes work assignments priorities for responsibility areas including nursing, Medical Assistants and ancillary staff. Responsible for management oversight of health center equipment needs, equipped examination rooms, infection control standards, CLIA and Radiology regulations, compliance with Standing Delegation Orders, and emergency events. Monitors the compliance with standards of care for on-site treatments and procedures, administration of medications, as well as timely and quality clinical workflows. Works with the practice administrator to ensure the clinical services team supports the overall operational and fiscal activities of the site to include staffing levels, budgets, financial and operating goals including meeting productivity and quality goals. Approves payroll and time off requests. Working in partnership with the practice administrator, Accountable for administering performance evaluations, recruiting, selecting, monitoring, and orientation of new hires. Responsible for coaching and counseling staff. Participates as an effective member of the site Triad management team to support implementation of continuous improvement activities. Routinely acts as the site lead in the absence of the Practice Administrator. Ensures timely assessments, interventions, and measurement for corrective action strategies when needed. Solves patient issues to ensure expected results. Assures quality of care philosophies and standards of care. Maximizes quality outcomes for patients’ services through the most appropriate utilization of systems. Serves on CUC committees as assigned. Adheres to state board of nursing and state nurse practice act requirements and to other governing agency regulations. Overview: In collaboration with patients, families (as defined by the patient) and staff across all disciplines and departments is responsible for performing selected clinical duties including administrative/clerical duties, basic patient care and/or performing administrative tasks associated with patient intake.
