Licensed Practical Nurse (LPN)

*Newly Increased Rates* Warsaw Meadows Care Center is seeking a compassionate Licensed Practical Nurse (LPN) to join our team at our skilled nursing facility in Warsaw, IN . The primary purpose of the LPN is to administer nursing care in a nursing home environment under the supervision of a Registered Nurse or other medical supervisors. Shifts Available: Full Time Day/Eve Shift (8 or 12 hours) Full Time 6a-6p/6p-6a Weekends Pay Range: Up to $37.50/hour PLUS eve/nights and weekend shift diff LPN Responsibilities Administer nursing care in a nursing home environment under the supervision of a Registered Nurse or other medical supervisors. Provide basic medical care including changing bandages, administering prescribed medications, and collecting specimens. Monitor resident conditions and report changes to the nursing team. Maintain accurate documentation of care and treatments provided. Work collaboratively with nurses and the interdisciplinary care team to ensure the highest quality of resident care. Qualifications Must possess a current Licensed Practical Nurse (LPN) license in the state of Indiana. Must be 18 years of age or older . Provides nursing care in accordance with Resident Care Policies and Procedures and ensures the safety and well-being of residents. Acts appropriately under the direction of the Charge Nurse and as an active member of the interdisciplinary care team. Assists residents with bathing, dressing, hygiene, and grooming in accordance with the established Care Plan. Ensures residents are out of bed according to the established Care Plan. Interacts with residents, family members, coworkers, and clinical and ancillary staff in a supportive, respectful, and professional manner. Performs additional duties as assigned in accordance with the resident Care Plan. CNA Qualifications High school diploma or GED preferred. Must be 18 years of age or older . Must possess an active Certified Nursing Assistant (CNA) certification from a state-approved training program. Certification documentation must be provided upon application.
