Job Description Be a part of a world-class academic healthcare system at UChicago Medicine as a Nurse Associate (RN) with our UroGyn Clinic. In this role you will partner with the rest of the clinical team to provide patients the highest quality of care. Eligible for a Sign-On Bonus up to $5,000! In this role, the Nurse Associate (RN) is responsible for coordinating diagnostic and therapeutic regimens for a specified group of patients, and maintains cohesiveness between clinical research, physicians, and patients. The Nurse Associate coordinates physician activities with referring physicians, nursing staff, hospital administration, ancillary services, support services, insurance companies and other departments/sections and agencies as needed, and assists the physician with a wide range of activities necessary to manage clinical research programs. In addition, the Nurse Associate functions as a liaison between physicians, patients, and families, assists physicians with bedside procedures, and provides patient care in the clinic with the physician as necessary. This role will predominately cover Urogynecology clinics and associated procedures. Experts at the University of Chicago Medicine provide a broad range of general and specialized obstetrics and gynecology services. From preventive gynecologic exams and prenatal care to minimally invasive surgery and high-risk pregnancy care, our team offers the latest advancements in reproductive health and wellness. Essential Job Functions: Conducts, responds to, follow up on, and triages patient phone calls to the physician as appropriate Assists physician in clinic or procedure area with patient examinations, diagnostic tests and/or therapeutic procedures. Continuously monitors vital signs of patients under conscious sedation throughout procedure Counsels patients concerning lifestyle modifications, medication regimens, side effects, and interactions using an appropriate psychosocial communication level Communicates to clinics coordinators follow-up appointments, ancillary and/or pre-operative testing and scheduling needed for patient Arranges patient admissions; coordinates ordering of tests/procedures with fellows, attendings, and other nursing staff Assists physicians in coordination of research programs by screening patients for acceptance into clinical research and outcome programs. Maintains office file notes, reviews charts, enters data for sectional demographic data base, assists in summarization and analysis of data Required Qualifications: Must have experience working in a community health setting Associate Degree in Nursing; BSN degree or higher preferred. Associate Degree Nurses are required to enroll in a BSN program within six (6) to nine (9) months of hire, and make progress toward and complete their BSN degree within 30 months. ADN nurses receive 100% tuition reimbursement towards a BSN degree from an accredited program. Must be an actively licensed Registered Nurse in the state of Illinois and Indiana is a plus Must have a minimum of two years' experience working as an RN Active BLS certification from the American Heart Association (AHA) Experience in Urogynecology is preferred. As requested, the HCW will provide input and technical guidance on issues related to the Health Promotion and Wellness (HPW) program. The HCW will coordinate and deliver reports to Health and Wellness teams. Consult and collaborate with already existing departments, command, and community programs to ensure appropriate utilization of resources to meet the patients’ health and wellness goals. The HCW will provide primary, secondary, and tertiary levels of intervention to promote effective education, self-management support, and timely health care delivery to improve the patient’s health and wellness efforts. The HCW will educate patients in understanding disease processes, impact of disease on overall health and wellness, and apply strategies including motivational interviewing and theoretical change model to improve health-patient engagement. The HCW will participate in a multidisciplinary team that assesses health promotion risk, analyzes health data, develops content for educational programs, drafts health promotion policy guidance and evaluates health promotions outcomes measures aimed to deliver improved patient care. The HCW will work with all clinical teams as a resource on patient education and disease prevention of behalf of all patients enrolled to the command. Improving patient care, quality of life and compliance are the focus to include the following: Pre-visit planning to ensure information and care completion prior to visit when possible. After visit summary review with patients whenever appropriate. Provide follow-up contact with patient to ensure compliance with recommendations. Educating and involving patients to improve their health-patient engagement. Assisting patients to set goals, progress and identify barriers when patient not meeting treatment goals. The HCW will conduct both formal/informal group and individually based educational and counseling interventions to include timely telephone follow-up consultation assessments and teaching to reinforce previous patient care plan goals and to ensure patient understanding. The HCW will evaluate achievement of learning objectives by the patient and/or family and provide appropriate follow-up in accordance with the patient’s treatment goals, referring patients to specialty and community services as appropriate. The HCW will assess educational needs and provide health promotion and disease prevention counseling for patients, family members and groups about modifications and/or specific nursing interventions. The HCW will employ adult learning behavior change theories and teaching techniques to conduct a variety of individual and group health promotion and disease prevention interventions. The HCW will provide expertise to implement and evaluate health promotion and disease prevention education programs for all populations. The HCW will communicate and coordinate health promotion and wellness-based programs among local leadership and target beneficiary populations. The HCW will coordinate special projects such as education-focused workshops, health fairs, informative displays, and programs for special groups. The HCW will complete documentation of all educational activities, services and/or encounters and ensure they are placed into patient medical records in compliance with established standards to ensure evidence of intervention and patient confidentiality.
