Monitors operative procedures by documenting proper patient positioning, breaks in sterile procedure, instrument, needle and sponge counts, preservation and labeling of all specimens, drugs and blood administration, and critical incidents during the operative procedure. Functions as a team member within the surgical department by assisting with gowning and gloving; procuring required medication, blood and supplies; coordinating involvement of other departments with the procedure; properly processing operative specimens. Maintains a safe and productive work environment by assisting with cleaning, maintenance, and re-supply. Successful completion of a training program and demonstrated competence in circulating nurse duties as determined by the Director of Surgical Services. Job Summary The Patient Care Advocate II (LPN/LVN) is a licensed, field-based member of WellBe’s interdisciplinary geriatric care team who delivers white-glove, high-quality, in-home care to frail, poly-chronic, elderly Medicare Advantage patients in the comfort of their own homes. This role combines patient-advocacy, care-coordination, and quality (HEDIS) responsibilities with clinical and telehealth visit-facilitation responsibilities. Practicing within the LPN/LVN scope of practice and at the direction of the assigned Provider, the Patient Care Advocate II conducts in-person patient visits, facilitates in-home telehealth consultations with an APP or Physician, captures and documents clinical data, supports patient education, and serves as the local-market connector for patient support services across the WellBe platform. The role models the WellBe mission and values, works closely with the provider and social-work teams, and functions as part of an integrated, interdisciplinary care team with no direct-report responsibility. The incumbent follows the rules and regulations of state and federal regulatory agencies and other certifying bodies for their region. Daily, local travel to meet patients and engage the community is an essential part of the role. Job Description Clinical Care & Visit Facilitation (LPN scope, at the direction of the assigned Provider) Facilitate in-home telehealth visits between the WellBe APP or Physician and the patient, including initial patient assessments, urgent visits and ongoing follow-up visits as appropriate to patient need. How you make a difference The Licensed Practical Nurse (LPN) plays a vital role in patient care by contributing to the nursing process of assessment, planning, implementation, and evaluation. Working under the supervision of RN(s), the LPN provides direction and guidance in patient education and activities that align with their education and demonstrated competencies. Key Responsibilities Under RN supervision, assesses patients' physical, psychological, and social dimensions in both the Health Care Unit and housing units, contributing to individualized treatment planning and consulting with RNs and staff as needed. Implements treatment programs directed by Health Care Practitioners, administering medications, including IV therapy (if certified), and performing diagnostic tests according to established protocols and procedures. Collaborates in controlled substance counts at the beginning of each shift, adhering to safety techniques, procedures, and standard precautions while responding to codes or health emergencies within standard guidelines. Assists in medical or minor surgical procedures, implements nutrition and therapeutic diet plans, and documents nursing encounters using SOAP charting, ensuring effective communication with nursing staff, physicians, and other relevant personnel. Attends mandatory staff meetings and training sessions, demonstrating a commitment to continuous professional development and adherence to policies and procedures in providing high-quality patient care. How you make a difference The Nurse Supervisor is responsible for the direct supervision of the health care delivery system within their assigned nursing unit. They collect health status data, diagnose patients, set goals, plan and implement care, and evaluate patient progress. The Nurse Supervisor also provides patient education and ensures that ancillary personnel are qualified to perform the services they provide. Key Responsibilities Review and ensure compliance of nursing plans, medical records, and referrals with policies and procedures. Monitor and facilitate progressive patient care through team approach and patient education. Assist DON in maintaining clinical skills of registered nurses and ensuring nursing actions are within scope of licensure. Identify and address potential problems, establish priorities, and propose corrective actions. Maintain unit cleanliness and coordinate patient care with other departments, while ensuring appropriate stock levels of medical supplies and equipment maintenance.
