Alliance Health at Baldwinville is hiring an experienced MDS Coordinator to manage resident assessments and care planning at our quaint, 95 bed, skilled nursing/long-term care facility. The MDS Coordinator Nurse will be responsible for, but not limited to: Completion of the OBRA, MDS and CAA’s per schedule Participate in the development and ensured completion of the Care Plan, identifying the individual needs of the residents. Facilitate weekly managed care meetings, participate in weekly care plan meetings, and other meetings as assigned. Assist with monitoring of resident documentation. Assist in ensuring C.N.A documentation is reflective of resident care.
