Pay: $110,000.00 per year, negotiable based on experience as an RN, Manager and UAS submitter Job description: Our Assisted Living Community Amber Court of Pelham Gardens, located in a beautiful residential area in the Bronx, is seeking a Registered Nurse for the Position of Wellness Director to join an amazing team. Responsibilities: Pre-Assessment and assessment of new admissions & re-admissions to the adult care facility Requires oversight of case management activities; case manager of record. Work with the health care team to assure that our residents receive the best quality care Implement policies and procedures related to the wellness of the residents, including infection control & prevention Perform and timely submit UAS Assessments via the Health Commerce System Organize resident care information and participate in Quality Improvement and other quality initiatives Provide ongoing guidance to the care team on Wellness processes, regulations and initiatives Perform duties in accordance with Assisted Living NYS DOH guidelines Perform duties in accordance with licensure as a Registered Nurse in New York. Requirements Candidate must be a Licensed Registered Nurse (RN) currently registered in New York State. Candidate must have the actual experience to Submit UAS Uniform Assessments via the Health Commerce System Candidate should have a warm and compassionate nature, able to interact with the geriatric population and staff in an empathetic manner Able to work as part of a team and contribute to the success of the company Prior experience in Assisted Living, Skilled Nursing or a related senior focused environment Prior management experience preferred Candidate must have completed UAS training Excellent computer skills needed.
