Responsibilities Work Schedule: Monday - Friday, 9 hours between 7:00am and 5:00pm, including a one hour lunch Core Duties: • Proactively identify candidates for CM or care coordination services using analytic tools, predictive analytics, and CM registries. Referrals may also come from the PCM/interdisciplinary healthcare team.• Wounded Warrior Case Manger -This case management (CM) role is dedicated primarily to the active duty wounded, ill, and injured (WII) population, but this role offers flexible capacity to support non-WII patients based on operational demand.• Identify individuals with chronic, catastrophic, complex, high utilization, high-risk, or high-cost health issues who would benefit from CM services and notify PCM teams, in accordance with DHA-AI 6025.39.• Provide CM and/or care coordination to these patients: Assess the patients’ needs, develop personalized care plans, and facilitate access to necessary healthcare services and community resources.• Provide education to patients and their families. Advocate for the patient’s needs to ensure timely and appropriate delivery of care.• Collaborate with PCM teams on CM activities. Provides routine updates and outcome metrics on patients enrolled in CM programs to PCM teams.• Provide behavioral health (BH) CM services and follow-up communications to eligible BH patients, in collaboration with the MTF Behavioral Health Lead, IAW DHA-AI 6025.39.• Document all CM or care coordination encounters within the patient’s EHR using proper coding and data capture IAW MHS coding guidance and DHA AI 6025.39. Document patient interactions, referrals, and care plans to maintain consistency and track progress.• Maintain a current registry of all patients receiving CM services. Coordinate with the PCM team staff members about patients moving into CM care (into the registries) and graduating from primary CM care back to the team (off the registries) and document such in an EHR note.• Provide CM/care coordination training, guidance, and mentoring to MTF personnel who utilize these services in their empaneled patients.• Participate in the MTF Medical Management, Population Healthy, Quality, and care coordination meetings as well as appropriate DHA or network webinars, meetings, or work groups. Qualifications Minimum Qualifications: * Degree/Education: Bachelor’s degree of Nursing. Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), the Commission on Collegiate Nursing Education (CCNE) Incumbent degree may be considered if lower than required. * Experience: Minimum of 24 months of full-time experience within the last 60 months working in case management, care coordination, or a related role with significant, direct responsibility for comprehensive client service planning and advocacy. This plan is based on using the 12-step approach described in the DoD Medical Management Guide.• Serves as a liaison with higher headquarters, TRICARE Regional Office, MTF national accreditation organization, professional organizations, and community health care facilities concerning Utilization Management. Participates in in-services and continuing education programs.• Participate in MTF Medical Management, Population Health, Quality, and Care Coordination meetings to ensure alignment and integration of UM activities and promote MTF health strategies. Shift Structure: Daily shifts will begin at 0630 and will be assigned as 8, 10, or 12-hour durations to meet variable surgical and patient care demands. Record all pertinent events taking place during the introduction of, maintenance of and emergence from anesthesia, including the dosage and duration of all anesthetic agents, other drugs, intravenous fluids and blood or blood components on all patients that have received anesthesia care. Record post-anesthetic visits, including at least one note describing the presence or absence of anesthesia related complications of the procedure. PATIENT All licensed health care providers shall provide patients with the utmost care and attention. All patients shall be assured of their privacy and personal dignity. Contractor personnel shall present a professional, conservative, and neat appearance. Contractor personnel shall report for duty in a professional manner, in appropriate attire befitting a health care setting, and having complied with socially acceptable standards of personal hygiene expected of health care workers. The Contractor will arrive to the start of the duty day earlier enough to prepare for the 1st case start to be on time (ex. in the OR room at 0730); and be flexible to be relieved or complete with the duty day within 15 mins before or after 1500 (or 1700 if on a 4-day /10-hour shift schedule). The Contractor shall ensure that all contract employees participate in the MTF orientation procedures for newly assigned medical employees to include regulations specific to their professional specialty, hospital and MTF policy and procedures. Contract employee training will include any in-processing and out processing, MHS Genesis, Joint Legacy Viewer, or job specific data training as required by the 99th Medical Group. The contractor will abide by medical staff bylaws upon acceptance of of privileges. Qualifications Minimum Qualifications: * Education: Masters or Doctorate degree in nursing.
