Overview This is a Per Diem position Wage Range: $48.18 - $81.97 + 15% Per Diem Premium The Home Health RN assesses, delegates, coordinates, and evaluates the nursing care provided. Ensures provision of quality care for selected groups of patients through utilization of nursing process, established standards of care, policies and procedures. The RN initiates the Plan of Care and necessary revisions and is responsible for the overall care of the patient. The Home Health RN is directly responsible to the Manager of Home Health and Hospice. Responsibilities Provide essential medical care and support to patients in their home setting, helping them to recover or manage chronic conditions and related symptoms. Conduct ongoing assessments of patient’s physical needs and or related symptoms and develop an individualized care plan based on the patient’s needs and goals of care. Clear communication and collaboration with the patient, family, caregivers and providers. Communicate observations, facts and treatment suggestions as an interdisciplinary team member, individually and through team meetings to ensure coordination of the plan of care. Provide education to patients, families and/or caregivers on disease and symptom management. Maintains timely, accurate and detailed records of patient assessments, care plans, treatment, and intervention response/outcomes. Advocate and provide support for the patient.
