Provides basic nursing care that includes actions that meet psychosocial needs and communication needs within the nursing assistant’s scope of practice under the direction of the bedside RN. 2. Ensures that patient’s needs are met at mealtimes and that patients receive their meals in a timely manner where applicable. 3. Completes documentation as described per policy, such as daily living activities, ambulation, meals etc, and other tasks as assigned. 4. Assists with gathering appropriate intake and output of patients and communicates with the bedside RN. 5. Ensures that patient daily hygiene needs are met, i.e. bathing, brushing teeth, changing of bed linen, etc under the direction of the bedside RN. 6. Performs basic patient care responsibilities considering needs specific to the standard of care for patient’s age. 7. Notifies appropriate licensed personnel when patient complains of pain or vital signs or intake and output are less than or greater then acceptable ranges. 8. Obtain and documents patient’s vital signs appropriately in the patient’s chart as requested by the bedside RN. 9. Demonstrates ability to communicate to the RN any observed changes in patient status and responses. Maintains awareness of the needs of the pediatric, geriatric and general patient population. 10. Demonstrates ability to handle patients safely and utilizes proper body mechanics and techniques; uses gait belt with transfer of fall risk patients. Ability to revise plan of care as indicated by the patient’s response to treatment and evaluate overall plan daily for effectiveness. 4. Ability to perform glucometer testing (point-of-care testing) per Clinical Laboratory’s and the patient care unit’s policies and procedures. 5. Ability to interpret results of waived tests; take appropriate action on glucometer test results. 6. Performs patient care responsibilities considering needs specific to the standard of care for patient’s age. 7. Knowledge of medications and their correct administration based on age of the patient and their clinical condition. 8. Follow the five 5 medication rights and reduce the potential for medication errors. 9. Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning, family is included in teaching as appropriate. 10. Demonstrates an ability to assist physicians with procedures and performs services requiring technical and manual skills. JOB SUMMARY: The Operating Room Circulator RN is responsible for the delivery and documentation of patient care during the intra-operative period. The RN will assess the patient’s needs, physiologically as well as emotionally, and plan nursing care accordingly. The nursing role during this phase focuses on providing quality patient care through familiarity with and adherence to policies and procedures, and standards of practice for perioperative nursing. She/he will also monitor compliance to established standards and policies and procedures for surgical scrubs. Performs the perioperative nursing role to include preoperative assessment of the patient, nursing care plan formulation, operative care plan implementation, and postoperative evaluation of the patient’s response/outcome. Also evaluates the effectiveness and appropriateness of nursing care provided with documentation of such. 2. Demonstrates understanding and utilization of principles of asepsis in all duties. 3. Assists in the setting up and performance of cases by assembly of equipment and supplies according to sterile technique. 4. Assists in the setting up and performance of cases by opening of sterile pack, instruments, and supplies according to sterile technique. 5. Prepares each patient for surgery by: positioning the patient in the appropriate position, preparing the patient’s operative site, confirming the procedure and site according to the surgeon’s history and physical and operating room team, and helping move the patient to and from the operating table. 6. Assists anesthesia personnel with the intubation/extubation of patients, the monitoring of fluid volume balance, the administration of blood/blood products, and the thermo-regulation of the patient. 7. Performs surgical counts (sponges, sharps, and instruments) and documents nursing care on legal records. 8. Handles specimens according to proper procedure. 9. Assists in the clean-up of the operating room following surgery, including proper disposal of used supplies and sharps. 10. Assumes responsibility for intra-operative patient safety related to positioning activities, the use of equipment, and the use of chemicals and solutions.
