Registered Nurse (RN) FT and PT 3P-11P We are seeking a dedicated Registered Nurse (RN) to join our team of healthcare professionals. Under the direction of the Director of Nursing, this role supervises nursing personnel and oversees daily nursing activities during assigned shifts. All duties must align with professional standards and federal, state, and local regulations to ensure the highest quality of care. Responsibilities Monitor the daily delivery of nursing care and evaluate staff performance in accordance with established policies and procedures. Plan and conduct regular shift staff meetings to identify and resolve concerns and improve services. Identify problems or potential issues in resident care and implement corrective action promptly. Provide written and verbal reports or recommendations to the Director of Nursing as needed. Assist in developing methods to coordinate nursing services with other resident services to support continuity of care. RN Supervisor We are seeking a dedicated Registered Nurse (RN) Supervisor to join our team of healthcare professionals. Under the direction of the Director of Nursing, this role supervises nursing personnel and oversees day‑to‑day nursing activities during assigned shifts. All responsibilities must align with accepted professional standards and federal, state, and local regulations to ensure the highest quality of care. Responsibilities Supervise clinical care planning and delivery to ensure coordinated, quality resident care, including review and updates of care plans. Assess residents upon admission and periodically as their condition changes. Conduct regular rounds to evaluate resident status and identify needs. Provide or assist with direct resident care when necessary, including activities of daily living, meal support, medication and treatment administration, and participation in leisure activities as appropriate. RN MDS Coordinator

RN MDS Coordinator (SNF Experience Needed) We are looking to hire an RN MDS Coordinator! Job Duties: · Tracks Medicare Customers to determine continued and appropriate Medicare eligibility and benefit period by determining skilled level of need · Performs concurrent MDS review to ensure appropriate RUGs category is achieved through the capture of appropriate clinical information. · Identifies opportunities to enhance reimbursement · Utilizes the PCC generated format to Communicate to the IDT team, the MDS assessment schedule to ensure timely facilitation of the Care Planning process · Oversee the day-to-day patient care, supervising, directing and developing nurse staff. · Coordinates the care plan according to regulatory requirements. · Ensure that resources are made available to patients and that patient care is delivered effectively and to a satisfactory standard. · Create the schedule for all Medicare and Medicaid. · Start Medicare coverage for newly qualified patients or send out denial letters and remain updated on changes in Medicare coverage and help determine documents needed for reimbursement.
