What is a community health center (FQHC)? The largest source of comprehensive primary care for underserved communities and populations. With an integrated care model that includes medical, dental, behavioral health, and pharmacy services, patients can access comprehensive, coordinated services often under one roof. Role Details Clinic Hours: Mon/Tues/Fri 8:30a.m.-5:00p.m. & Wed/Thurs 8:30a.m.-8:00p.m. Your Schedule: 4 days per week 8:30-5:00 and choosing Wed or Thurs to work one later shift 8:30-8:00 EHR: OCHIN Epic Benefits Competitive Salary: $29.42-34.00 hourly depending upon experience Licensing and Professional Subscriptions: We cover professional dues and fees & offer continuing education Comprehensive Insurance: Medical, dental, vision, life, accidental death and dismemberment, short-term disability, long-term disability, and flexible spending available. With an integrated care model that includes medical, dental, behavioral health, and pharmacy services, patients can access comprehensive, coordinated services often under one roof. Role Details Clinic Hours: 7:00 a.m.-6:00 p.m. Monday-Thursday & 8:00 a.m.-5:00 p.m. Fridays Schedule: flexible 4-day work week within clinic hours seeing approximately 18-22 patients per day. Provider team includes: 9 providers with 1 CMA & 1 RN/LPN per provider Operatories/Exam Rooms: 18 EHR: Epic Benefits Student Loan Repayment Programs: Eligible for $50,000 in student loan repayment in exchange for 2 years of service through state & federal programs Competitive Salary: $30-38 per hour negotiable depending on experience + quarterly team production incentive. This role performs primary care to the community providing a mix of well visits, acute care, & chronic disease management throughout the age spectrum. What is a community health center (FQHC)? The largest source of comprehensive primary care for underserved communities and populations. With an integrated care model that includes medical, dental, behavioral health, and pharmacy services, patients can access comprehensive, coordinated services often under one roof. Role Details Clinic Hours: Mon, Tues, Fri 8am-5pm & Wed, Thur 8am-8pm Schedule: 5 day work week with 1 evening per week (11:30a-8p) Sees on average 12-14 patients per day Provider team includes: 5 physicians (including the Co-Medical Director), 6 Nurse Practitioners, 2 Physician Assistant.
