Your Job Qualifications • Currently licensed as an RN in the state practiced. • Associate or Bachelor Degree from an accredited nursing school preferred. • Five years in long-term or acute health care preferred. • At least 2 years nursing education experience required. RESPONSIBILITIES • Assesses the training needs of nursing staff to develop, implement, evaluate and document staff development programs including General Orientation, Job Specific Orientation, In-Service, and Continuing Education Programs for nursing department personnel. • Identifies, plans, implements, directs and evaluates educational program within the facility based upon individual facility needs, resident needs, and in compliance with State and Federal regulations; implements these programs on all shifts. • Responsible for the planning, coordinating and conducting required in-service programs and/or special training sessions in compliance with applicable federal, state and local laws and/or corporate requirements. • Collaborates with the Director of Nursing (designee) to complete periodic needs assessment identifying ongoing learning needs of staff and assists in developing and conducting educational programs to meet the identified needs. • Maintains appropriate documentation of educational programming, each individual employee’s attendance at educational programs and competency validation as needed. • Develops implements and maintains a written annual staff development plan, which ensures a coordinated program for staff education for nursing personnel. • Coordinates job specific orientation to familiarize and train new nursing department hires to the expectations of the job. • Acts as preceptor, coach, and resource person to assist nursing department employees to successfully complete the job specific orientation. • Monitors staff for compliance with OSHA mandates and facility policies on workplace safety. Proactively develops procedures and incentives to promote workplace safety and safe work practices. • Supervises development and delivery of in-service education to equip nursing staff with sufficient knowledge and skills to provide compassionate, quality care and respect for resident rights. • Develops positive employee relations, incentives, and recognition programs. Promotes teamwork, mutual respect, and effective communication. • Maintains up to date personnel files with each employee’s educational information. • Assists in planning, coordinating, and conducting required training to assure compliance with Quality Improvement program in conjunction with departments. • Establishes an annual budget that will provide necessary resources to achieve program objectives and cost efficiencies. • Assists with demonstrations to acquaint staff with new equipment and to review established equipment. • During training provides supervision and oversight of employee to ensure skills are performed as outlined in nursing procedures • Review employee performance during skills observations. • Ensures employee satisfactorily completes orientation and other assigned training sessions. Certified Nurse Aide / CNA PRN

POSITION DESCRIPTION : A nursing assistant responsible for providing routine nursing care in accordance with established policies and procedures and as may be directed by the Charge Nurse, RN Supervisor, Director of Nurses or Administrator, to assure that the highest degree of quality resident care can be maintained at all times. QUALIFICATIONS Currently enrolled in a school for nursing assistants or have a license as a certified nursing assistant. A minimum of 6 months experience as a certified nursing assistant or clinical experience while enrolled in a nursing assistant certification class. Must possess a sincere desire to serve the ill, aged and infirm. Ability to read, write and speak the English language. Possess the ability to deal tactfully with personnel, residents, family members, visitors, and the general public. Willingness to work harmoniously with other personnel. Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level they are currently functioning. Must be in good general health and demonstrate emotional stability. Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility. GENERAL DUTIES AND RESPONSIBILITIES: GENERAL Perform all duties as assigned and in accordance with facility’s established policies and procedures, nursing care procedures and safety rules and regulations. Make resident rounds at the beginning of each shift and every two hours thereafter to administer quality nursing care. Verify the identification of a resident before administering nursing care or serving food, to assure that appropriate care is being provided to the correct resident. Report missing ID bands to admission coordinator immediately. Make beds, clean bedside and overbed tables. Assure that water pitchers are cleaned, filled with fresh drinking water and within the resident’s reach. Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. Transportation Aide (CNA)

Your Job Responsible for coordinating transportation and transportation of residents to and from physician appointments in a timely manner and under the supervision of licensed nursing personnel. QUALIFICATIONS • Valid Driver's Licensure for state in which you are applying. • Successful completion of State Approved Nurse Aide Training and Competency Evaluation Program; Name must appear on State Registry as Nurse Aide in good standing. • Current BLS/CPR certification (All SNFs; ALFs as required by state) RESPONSIBILITIES • Organizes transportation of residents to complete required appointments based on priority of resident needs. • Transports resident to and from inpatient and outpatient programs and to and from appropriate facilities and residences. • Develops driving schedule based on resident’s scheduled medical appoints and program destination depending on resident appointment needs; Provides transportation of current or new residents from hospital after discharge. • Provides report, information, and assessment of resident’s behavior or change of condition during transportation upon return to center; Reports changes of abnormal findings in resident status such as consciousness, pain, bleeding and vital signs that occur during the lift or transporting; Observe residents carefully and reports changes in condition to clinical supervisor. • Transports medical records for signatures to medical offices. • Maintains a high level of safety following safety policies and procedures while in Center and transporting residents. • Maintains van in a neat and clean condition at all times; Maintains van in good working condition; Ensures that routine maintenance on vehicle is performed, logs mileage, and that vehicle is clean. • Assist clinical staff with safe lifting, transferring, and/or transporting resident as specified by the physician orders and/or clinical supervisor; Assist in the care of residents requiring moderate to total assistance in moving. • Establishes positive communications with outside facilities, hospitals, physicians' offices, and /or specialty clinics to ensure proper services for residents are maintained and scheduled appropriately. • Promptly reports any scheduling changes to the DON; nursing staff and ancillary service providers, responsible parties and takes follow up action as necessary. • Provide adequate hydration to residents at regular intervals each day according to specific resident needs. • Protect the personal property of the residents of the facility. • Other duties, responsibilities and activities may change or assigned at any time with or without notice.
