Job Summary: As an LPN/RN MDS Coordinator, you will be responsible for coordinating the Minimum Data Set (MDS) assessment process, ensuring accurate and timely completion of assessments, and maintaining compliance with Medicare and Medicaid regulations. You will work closely with our care team to develop and implement individualized care plans that meet the unique needs of our residents.
