CNAs play a vital role in maintaining a safe, clean, and supportive environment while promoting resident independence and wellbeing. Work is performed collaboratively within an interdisciplinary team focused on delivering compassionate, person-centered care to all residents of the Health Care Center. • Provide direct, person-centered care to residents under the supervision of licensed nursing staff in accordance with individualized care plans, state and federal regulations, and facility policies. • Assist residents with daily living activities including bathing, dressing, grooming, toileting, oral hygiene, mobility, and feeding while promoting independence and dignity. • Observe, monitor, and report changes in residents’ physical, emotional, or behavioral condition to the charge nurse in a timely manner. • Complete required documentation and charting accurately and promptly, including meal intake, activities of daily living (ADLs), and behavior or condition changes. • Provide assistance with restorative and rehabilitative programs such as range-of-motion exercises, ambulation, and positioning. • Ensure resident comfort by repositioning, adjusting bedding and equipment, and maintaining a safe and sanitary living environment. • Assist with transporting residents to and from meals, activities, and therapies. • Participate in resident mealtime routines by preparing trays, serving meals, feeding residents as needed, and observing dietary restrictions and swallowing precautions. • Monitor safety during meals to prevent choking and follow diet orders and physician instructions. • Perform routine housekeeping tasks on the unit such as making beds, changing linens, cleaning wheelchairs and equipment, and maintaining neat and organized resident areas. • Respond promptly to call lights, alarms, and resident requests, ensuring needs are met efficiently and respectfully. • Record and report vital signs, weights, intake/output, and other relevant resident data per facility protocol. • Support infection prevention practices and follow all safety, sanitation, and universal precaution guidelines, including use of Personal Protective Equipment and enhanced barrier precautions . • Assist nursing staff in emergency situations, including falls, medical incidents, and resuscitation measures as trained and directed. • Participate in staff meetings, care conferences as needed, and in-service education sessions to maintain required certifications. • Maintain supplies and equipment; report maintenance or safety concerns promptly to supervisory staff. • Promote resident rights, privacy, and confidentiality at all times. • Perform other related duties as assigned to ensure the continuity and quality of resident care. The Registered Nurse (RN) provides professional nursing care and clinical oversight to residents of Rocky Knoll Health Care Center in accordance with the nursing process, physician orders, and State and Federal long-term care regulations. This position is responsible for assessing resident conditions, administering medications and treatments, supervising and directing licensed and unlicensed nursing staff, and coordinating care with the interdisciplinary team to support resident safety, dignity, and quality of life. The Nurse Manager provides daily leadership, supervision, and clinical oversight to ensure residents receive safe, high-quality, person-centered care. This working-manager role oversees staffing, workflow, and clinical operations while supporting nursing staff through guidance, education, and direct involvement in care as needed. The Nurse Manager responds to changes in resident condition, leads investigations and follow-up on incidents, and promotes consistent adherence to facility policies, regulatory standards, and best nursing practices.
