This role is essential in ensuring residents receive individualized attention, coordinated services, and support with daily living needs. As the Assisted Wellness Director, you will work closely with clinical staff, residents, and families to help develop and implement care plans, monitor resident well-being, and promote a safe, supportive, and engaging environment. Key Responsibilities Collaborate with interdisciplinary team members to develop and implement individualized care plans based on resident needs and preferences Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support as needed Conduct routine wellness checks and observe for changes in resident condition, reporting concerns to the appropriate clinical staff Maintain accurate and timely documentation, including care plans, progress notes, and vital signs in accordance with facility policies Communicate effectively with residents, families, and care team members regarding updates and concerns Support resident independence while ensuring safety and comfort Participate in quality improvement initiatives to enhance resident care and service delivery Assist with coordinating services and resources to support resident needs Qualifications High school diploma or equivalent required Previous experience in healthcare, assisted living, or senior care preferred, but not required Knowledge of senior care practices and resident support preferred Strong communication, organization, and interpersonal skills Ability to work collaboratively within a team environment Current CPR/First Aid certification preferred Ability to work flexible schedules, including weekends and holidays as needed Commitment to providing compassionate, resident-centered care Benefits Competitive compensation 401K Health, dental, and vision insurance Paid Time Off (PTO) Supportive and collaborative work environment Grow With Us At Heritage Grove Estates, we are committed to the development of our team members. Job Title: Certified Medication Aide (CMA) Position Summary: Heritage Grove Estates is seeking a skilled and compassionate Certified Medication Aide (CMA) to join our care team. In this role, you will provide essential support in medication administration while assisting residents with daily living activities. The ideal candidate is a dedicated advocate for residents and their families, committed to delivering safe, high-quality, and person-centered care in collaboration with the interdisciplinary team.
