Position: The Advanced Practice Provider (APP) provides high-quality, patient-centered primary care to Wayne HealthCare patients. The APP assesses, diagnoses, treats, educates, and manages patients within their education, training, certification, licensure, and approved scope of practice. This position is open to qualified Nurse Practitioners (NPs) and Physician Assistants (PAs), including qualified new graduates. Requirements: Master’s degree from an accredited Nurse Practitioner or Physician Assistant program. Current, unrestricted Ohio NP/APRN or Physician Assistant license. Current national board certification appropriate to specialty. Ohio Certificate to Prescribe, as applicable. Current BLS/CPR certification. Ability to obtain and maintain required hospital, payer, Medicare, Medicaid, DEA, and Wayne HealthCare credentialing. Primary care, family practice, internal medicine, urgent care, or comparable experience preferred. One year of NP/PA experience preferred, but not required; qualified new graduates may be considered. Responsibilities: Obtain patient histories and perform comprehensive or focused examinations. Assess, diagnose, and manage acute, chronic, and preventive health conditions. Develop individualized treatment plans and prescribe medications within applicable laws and scope of practice. Order, perform, and interpret laboratory and diagnostic testing. Monitor treatment effectiveness, medication response, and patient adherence. Provide preventive care, wellness counseling, and patient education. Provide telephone consultation and clinical triage as assigned. Communicate diagnoses, test results, treatment plans, and follow-up recommendations. Identify conditions requiring physician consultation, specialty referral, or higher levels of care. Coordinate care with physicians, specialists, hospitals, and other healthcare professionals. Maintain accurate, timely medical records and documentation in the electronic medical record. Comply with applicable laws, regulations, payer requirements, HIPAA, and Wayne HealthCare policies. Participate in quality improvement, patient safety, chart review, and performance improvement activities. All other duties as assigned

Case Management - RN Case Manager

This is a part-time day shift, 32 hours per week position for an RN Case Manager with hospital background. Weekend rotation is required for every 4 th Saturday. We are in search of a Case Manager to enhance the quality of patient management, maximize satisfaction and promote cost effectiveness. The successful candidate will be accountable for the full patient care cycle. Qualifications: Currently licensed to practice in the State of Ohio as a Registered Nurse ASN required, BSN degree preferred Experience in case management is a plus Excellent knowledge of case management principles Effective communication skills Problem solving skills and ability to multi-task. Med/Surg/ICU experience Responsibilities: Case Manager duties include assessing, planning, implementing, monitoring, and evaluating actions required to meet the client’s health and discharge services needed. Coordinate and provide care that is safe, timely, effective, efficient, equitable and client centered. Arrange services necessary for a safe discharge ie: home health, SNF, outpatient services. Handle case assignments, review cases daily and provide concurrent review Submit necessary cases to the appropriate insurance company for authorization of stay. Develop effective working relations and cooperate with medical team throughout the entire case management process. Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction. Record case information; complete accurately all necessary forms/documentation. Promote quality and cost-effective interventions and outcomes. Ability to multi-task, problem solve with excellent written and verbal skills. Ability to work effectively in a self-directed role. Prior authorizations Run daily reports. Communicate with insurance companies to obtain authorization for proper reimbursement. Adhere to professional standards as outline by protocols, rules, and regulations. All other tasks as assigned.

Qualifications: Currently licensed to practice in the State of Ohio. BSN preferred Current BLS Certification required. 2 years of OB RN experience required. Post-Partum, Obstetrical, Labor & Delivery experience required. NRP and ACLS Certification preferred, BLS completion required. 1-year Medical/Surgical experience in acute care setting preferred. Skills and Ability: Interpersonal and analytical skills necessary to function under the current standards of nursing practice. Physically able to perform all the functions of the position. Fetal monitoring course required within six months, External Fetal Monitoring Certificate completion within 2 years of hire. NRP certification required within six months.
