Job Responsibilities

  • Receiving orders and care plans from a virtual care team for patients during their acute phase of illness
  • Working with virtual nurses and hospitalists to identify interventions and other needs for patients in the hospital at home setting
  • Providing comprehensive daily patient assessments and ongoing support for advanced care planning
  • Administering vascular access care, wound care, infusion, and other nursing interventions
  • Providing patients with health education and coaching regarding treatment decisions
  • Making evidence-based clinical decisions that align with scope of practice, medical necessity guidelines, practice standards, industry standards, best practices, contractual requirements, and legal directives
  • Leading clinical operations and providing medical management across the continuum of care (assessing, planning, implementing, coordinating, monitoring, and evaluating)
  • Participating in home visits and transition of care management for patients
  • Obtaining, documenting, and reporting information about specific symptoms and questions
  • Promoting patient engagement through empathetic care that develops rapport and builds trusting relationships

Qualifications and Skills

  • Active New Jersey state Registered Nurse (RN) license
  • At least 1 year of experience in an acute care, in-patient, hospital setting
  • Ability to place a peripheral line with experience maintaining and accessing PICCs and midlines
  • CPR for Healthcare Providers Certification from the American Heart Association
  • Strong clinical and assessment skills
  • Ability to travel to multiple patients' homes during shifts and to work independently
  • Current influence vaccination and Hepatitis B and MMR records
  • Updated resume
  • Ability to lift a minimum of 50 lbs
