🩺 About the Role: 

As a Licensed Practical Nurse (LPN), you’ll be a key contributor to our patient care team, delivering skilled nursing services under the supervision of a Registered Nurse and in alignment with physician orders. Your role is essential in supporting individualized care plans, administering treatments, and ensuring a safe and healing environment for every patient. Position Highlights at a Glance: 

  • Caseload: ~7 patients 
  • Average Patient Length of Stay: 10–14 days 
  • Conditions: Stroke, brain & spinal cord injuries, cardiac, ortho, complex medical, and more 
  • Team Approach: Work closely with PT/OT/ST, physicians, case managers, respiratory therapy & more 
  • Advanced Care: IVs, trachs, wound care, mobility support, lines, tubes, and drains 
  • 12-hour shifts, 3 days/week + rotating weekends/holidays (with added pay)

  • LPN License (State of Wisconsin)
  • CPR required | ACLS & BLS preferred 
  • 1+ year of LPN experience preferred (Acute care a plus)

Physical Requirements Over the Course of a Shift:

  • A significant amount of standing, walking, bending, reaching, lifting, pushing, and pulling, often for prolonged periods of time.
  • Both gross and precise motor functions.
  • Lifting/exerting of up to 50 lbs.
  • Possible exposure to bodily fluids.
  • Visual acuity required for patient assessment and documentation of care.
  • Acute hearing required for accurate patient assessment.
  • Sufficient manual dexterity to operate equipment and computer keyboard.

Job Summary Provides support for care management/care coordination activities and collaborates with multidisciplinary team coordinating integrated delivery of member care across the continuum.
