Responsibilities

  • Provide skilled nursing care in patients' homes under the supervision of a Registered Nurse.

  • Follow and carry out individualized Plans of Care established by the supervising RN.

  • Perform skilled nursing visits and complete accurate, timely clinical documentation.

  • Monitor patient status and promptly communicate changes to the supervising RN and healthcare team.

  • Educate patients and caregivers on medications, treatments, disease management, and self-care techniques.

  • Reinforce the Home Health Aide plan of care and collaborate with the interdisciplinary team to ensure quality patient outcomes.

  • Participate in case conferences, staff meetings, and quality improvement activities.

  • Promote safe, compassionate, high-quality patient care while helping patients remain safely at home.

Qualifications

  • Graduate of an accredited Licensed Practical Nursing (LPN) program.

  • Current, unrestricted Ohio Licensed Practical Nurse license.

  • Current CPR certification.

  • Minimum of one year of LPN experience required.

  • Home Health experience is preferred but not required.

  • Valid driver's license, reliable transportation, and auto insurance.

QUALIFICATIONS & ATTRIBUTES: Licensed Practical Nurse with current license in the state of employment. Minimum of one year of experience as a LPN in an acute care setting is required; two years is preferred. Home care experience is desired. MAJOR AREAS OF RESPONSIBILITY: Provides skilled nursing care in a patient’s home as ordered by the attending physician. Performs care by following order sets and goals made by RN. Observes and monitors patient conditions. Notifies RNCM of any changes in patient condition. Administers medication as prescribed by the physician. Counsels patient and family on the disease/injury processes and how to manage. Incorporates patient and family in development of plan of care. Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions. Listens to patient and family members. Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines. Documents all aspects of treatment, assessments, and patient education. Maintains active LPN license in state of employment. Communicates with scheduler any changes outside the normal. Participates in all on-call requirements and case conferences. Attends in-service trainings and mandatory agency meetings. Maintains a professional appearance as a representative of the company. Understands and practices agency policies and procedures. Other duties as assigned. ESSENTIAL FUNCTIONS: Drives to patient’s primary location per scheduled visit. Daily attendance at assigned visit locations. Documents all aspects of subsequent visits within 24 hours of visit to meet Medicare guidelines. Completes and submits all required documentation within specified company requirements. Knowledgeable with providing skilled care within the scope of practice for a Licensed Practical Nurse. Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications.
